About the Sliding Fee
This system ensures the cost of your visit matches what you can actually afford, based on your income and family size. Callen-Lorde maintains this scale so the amount you owed for services are adjusted based on your ability to pay.
About Eligibility
All patients, regardless of insurance status, are eligible for services at Callen-Lorde on a sliding fee basis.

About Lab Fees
Please note that all laboratory tests collected during your visit will be processed by an external entity.
For billing questions related to the tests, use MyChart to a send a message to Billing. Financial counselors will review the message and contact you directly to assess your eligibility for a lab fee discount.
If you are not signed up for MyChart, speak with our front desk to enroll or have your information forwarded to the financial counselors. The counselors will reach out directly to provide guidance.
About Federal Poverty Level (FPL) Categories
Ask our front desk if you need help determining your FPL category. This is based off of your household size and annual income. Your household includes yourself, your partner (if you file taxes together) and any additional dependents.
Need to update your category?
If your income has changed, bring proof of income to any front desk to update your sliding fee scale discount. This can’t be done over the phone or on MyChart.
No proof of income? Ask for a self-attestation form instead.
