Patient Eligibility
Callen-Lorde Chelsea, Brooklyn and the Bronx, including the Health Outreach to Teens (HOTT) program are currently accepting new primary care patients.
If you are registering for the Health Outreach to Teens (HOTT) program (ages 13-21), please click here or call 212-271-7212 and speak to a HOTT staff member. Establishing Adult Medical Care
Step 1. Make an appointment
You can now make your first appointment in Manhattan, Brooklyn, or at the Bronx online! Click HERE to schedule your appointment.
If you have insurance, we will review your insurance information upon submission to verify your payor and selected provider are in network, and will follow up with you if additional information is needed.

Step 2. Complete Registration E-Check-in
48 hours before your scheduled visit, you will be prompted to complete your Registration information via the E-Check-in process on MyChart. Please provide all information requested to the best of your ability.
Step 3. Keep your intake appointment
Confirm and attend your first appointment to establish your care! If you are unable to complete the E-Check-in via MyChart, you may register in person before your appointment. You may print the Registration and Consent forms [ENG , SPN , RUS] or the front desk can provide them when you arrive at the clinic.
Please arrive at least 30 minutes before your scheduled appointment time to complete final steps of your registration. When you arrive at the clinic, please make sure you have:
· Your government-issued photo ID, such as a driver’s license, NYC ID or passport.
· Your insurance card, if you have health insurance.
Large print registration forms are also available in English and Spanish.
