Become a Patient at Callen-Lorde

Callen-Lorde Chelsea, Brooklyn and the Bronx are currently accepting new primary care patients and patients aged 13-21 who want to be part of our Health Outreach to Teens program. We are not currently taking new patients for dental or behavioral health care.

Establishing Care in 3 Easy Steps

Step 1: Make an Appointment

Call 212-271-7200 to schedule an appointment in Chelsea, Brooklyn or the Bronx. Our call center representatives are ready to schedule your new patient appointment at any of our three sites.

Step 2: Send Documents

At least 48 hours before your scheduled visit, please email your completed Registration and Consent forms, ID, and insurance card. Please include your appointment details in the body of your email.

Please download and complete our registration forms, which are available in English, Spanish and Russian. Email your forms to registration@callen-lorde.org.

If you are interested in our Health Outreach to Teens (HOTT) Program for patients aged 13-21, please email to HOTTregistration@callen-lorde.org. 

Step 3: Keep Your Intake Appointment

Confirm and attend your first appointment to establish your care! If you are unable to complete and submit the registration forms via email, you may complete them in person before your appointment.

Please plan to arrive at least 30 minutes before your scheduled appointment time. When you arrive at the clinic, make sure you have

• Your government-issued photo ID, such as a driver’s license, NYC ID or passport 

• Your insurance card, if you have health insurance 

• Your completed registration forms if you’ve printed them out instead of emailing them
  

Large print registration forms are also available in English and Spanish.
