Elder Care

Many New Yorkers face unique healthcare challenges as they age, from new medical conditions to difficulty enrolling in Medicare or Medicaid. Callen-Lorde understands these issues—and that’s why we created the Elderflower Program in 2022. 

Elderflower helps patients live authentically and age healthily. Below you’ll find information about the scope of elder care services at Callen-Lorde and how you can access important services.  

Our services available for older adults  

PAWH (People Aging with HIV) or PrimeTime+ 

PAWH provides medical case management, service plans and referrals to patients over 50 living with HIV. We hold a series of social support groups for participants, and peers help them navigate their healthcare needs. 

Eligible patients must live in New York to participate in PAWH. To hear more about the program or enroll, contact your primary care provider who can facilitate a referral. 

Healthy Aging accreditation 

Callen-Lorde has been accredited as an Age-Friendly Health system by the Institute for Healthcare Improvement since 2023. Age-friendly systems aim to follow an essential set of evidence-based practices, cause no harm and align with what matters to older adults and their family caregivers. Medicare enrollment 

Callen-Lorde case managers can guide you through the Medicare enrollment process if you’re newly eligible, unsure about your options or need help completing your application! 

Most people become eligible for Medicare at age 65—or earlier if they have certain disabilities or end-stage renal disease. The program’s open enrollment period, where you can enroll in or change your coverage, runs from October 15 to December 7. 

To enroll you’ll need proof of age or disability, a Social Security Number, and documentation of U.S. citizenship or lawful residency. Come into one of our clinics to work with a case manager. 

Healthcare proxies  

Callen-Lorde suggests all of our patients, but especially those over 50, fill out healthcare proxy forms. These documents will help us to understand who you would like to reach out should you be unable to speak for yourself. In times of medical emergencies, these can also be shared with emergency room staff.

Cardiac care 

Callen-Lorde has a cardiologist from Northwell Health, the state’s largest hospital system, on site twice a month. Speak to your primary care provider to book an appointment. 

HIV and Aging Committee of the Community Advisory Board 

At Callen-Lorde, all patients have the opportunity to get involved with the organization and provide input to our providers. You can apply to give input into the Healthy Aging Program specifically through this link!
