Become a Patient at Callen-Lorde

Patient Eligibility 

Callen-Lorde Chelsea, Brooklyn and the Bronx are currently accepting new primary care patients and patients who want to be part of our Health Outreach to Teens program. We are not currently taking new patients for dental or behavioral health care.

Before coming in 

Prior to your scheduled appointment, please fill out our registration forms which are available in English, Spanish or large print in English and Spanish. Email it to registration@callen-lorde.org. 

When you arrive at the clinic, make sure you have 

• Your government-issued photo ID, such as a driver’s license, NYC ID or passport 

• Your insurance card, if you have health insurance 

• Your completed registration forms if you’ve printed them out instead
