COVID-19 VACCINE RECORDS

NYC is currently requiring proof of COVID-19 vaccinations which are now mandatory for entry to indoor eateries, gyms, travel, and entertainment venues.

To find and download your vaccine record, visit My Vaccine Record. You can use your mobile phone number, email address, or IDNYC number to search for your immunization record in the application.

Vaccination cards are given to each patient at the time of initial vaccination and is qualifying proof of immunization. Please note that Callen-Lorde does not replace COVID vaccination cards but will issue an official CIR (Citywide Immunization Registry) or EHR (Electronic Health Record) proof of immunization. Damaged or incorrect vaccination cards can be brought into the clinic to be exchanged for a corrected one.

Other forms of valid proof include: Excelsior Pass and the Key to NYC Pass.
