ASSOCIATE DIRECTOR, MEDICINE

Position Summary

The Associate Director of Medicine (ADM) collaborates with the Director of Medicine in providing clinical and administrative leadership and vision to Callen-Lorde’s medical practice, particularly to the primary care service. The ADM assists the Director of Medicine in managing the professional, organizational, and administrative aspects of the quality of care provided to Callen-Lorde patients. In addition this role will provide direct supervision to a group of Medical Providers and support the orientation and onboarding of all new Medical Providers. This role will also provide input into the development of electronic medical record templates as well as the overall functionality and regulatory compliance of the EMR. This position will also provide direct quality, sensitive patient care on a part-time basis (five sessions weekly) documenting care promptly and following charting protocol and applicable NYS licensing requirements. The ADM will maintain a small panel of primary care patients, provide coverage when other Medical Providers are out of the office as well as working in the Urgent Care and Sexual Health Programs.

Work Schedule/Salary/Benefits

The Health Center is open Monday through Saturday. This position requires weekly evening and occasional weekend and on call coverage. Qualifications

  • York State-Licensed Physician Board Certified in Internal Medicine, Family Practice.
  • Certification as an HIV Specialist highly preferred.
  • At least three years of clinical experience in an outpatient medical setting required; Experience providing primary care and HIV/AIDS- related care is required.
  • Prior supervisory experience is preferred but not required.
  • Commitment to community based health care services and the mission of Callen-Lorde.

DENTAL ASSISTANT

Position Summary

Callen-Lorde is seeking exceptionally friendly, organized individuals with excellent skills to join our staff as Dental Assistants. In this position you will provide clinical support to dentists and hygienists, including preparation of patients, supplies, and rooms; chair side assistance; radiology and laboratory assistance. Additionally, you will be responsible to ensure appropriate and efficient patient flow for the Dental Providers.

Work Schedule/Salary/Benefits

The Health Center is open Monday through Saturday, hours of operation and related shifts range from early morning to late evenings. Evenings and Saturdays are required. GYNECOLOGIST – PART-TIME

Position Summary

Callen-Lorde is seeking an experienced, dedicated, gynecologist to join our team on a part-time basis. This role will provide gynecological care to a diverse female bodied patient population including patients who identify as lesbian, bisexual, heterosexual, gender non-conforming, and transgender. The ideal candidate will provide routine and urgent GYN care, perform colposcopies and high resolutions anoscopies (HRA), as well as participate in and expand our alternative insemination program. This candidate will be the collaborating physician for mid-level women’s health providers.

Work Schedule/Salary/Benefits

The Health Center is open Monday through Saturday day and evening. We are seeking someone who can commit to working approximately a 15 hours per week schedule as a per diem.

Qualifications

  • New York State Certification/Licensure and current New York State Registration.
  • Board Certification in Gynecology.
  • Certification and experience in high resolution anoscopy required.
  • Demonstrated knowledge of LGBT health related concerns.
  • Experience in sexually transmitted infection screening, treatment, and education.
  • Experience in providing care to diverse populations, including transgender patients adults and adolescents, preferred.

HEALTH HOMES OUTREACH NAVIGATOR

Position Summary

Callen-Lorde is seeking a dedicated, flexible individual to join our staff as a Health Homes Outreach Navigator. The outreach navigator is responsible for providing outreach and engagement services, case management, patient advocacy, semi-annual psychosocial assessments, intake assessments and crisis intervention services to individuals accessing medical or mental health services and are living with a chronic illness. The Health Homes Outreach Navigator is responsible for ensuring the HIV Special Needs contract requirements are met; as well as, perform outreach and intake services to any patients assigned to Callen-Lorde’s Health Home from Mount Sinai. The Outreach Navigator will work closely with medical, mental health, and HIV outreach workers to ensure that the full continuum of care needs of our patients are identified and appropriate services accessed, coordinated, and monitored. The Outreach Navigator will offer administrative support to the Director of Care Coordination and the Manager of Intensive Care Coordination by performing data collection and review, filing organization, facilitating legal services, purchasing office supplies, and additional special projects.

HOTT YMSM (Youth Men Seeking Men) PROJECT COORDINATOR

Position Summary

The HOTT YMSM Project Coordinator will establish and coordinate all aspects of the YMSM event venue cultivation and outreach in the MSM youth community. Primary responsibilities include establishing on-going working relationships with party/event promoters targeting YMSM in various venues throughout Brooklyn, Queens, Manhattan and the Bronx, that would enable the Mobile Medical Unit (MMU), YMSM Prevention Counseling and clinical staff to provide clinical services, sexual health services, prevention counseling, testing and referral services to young MSM between the ages of 13 through 24.
The Coordinator will provide HIV testing and STI screening, health services and assessment/risk reduction counseling services to event participants seen on a walk-in basis. Responsibilities will also include on-going data collection, reporting, and timely submission of data/service encounters. In addition, the project coordinator will participate in active recruitment and community outreach, education and marketing activities, related to the provision of YMSM Health, sexual health services, through community events, vogue parties, the Ball & House scene and other venues where young MSM within the target demographic congregate in the New York metropolitan.

Work Schedule/Salary/Benefits

Callen-Lorde HOTT program is open Monday through Friday. Some evening and Saturday hours will be required. SBH Behavioral Health Specialist – LMSW/LMHC/CASAC(Part-time)

Position Summary

The part time SBH Behavioral Health Specialist will participate in all aspects of the agency’s Prevention and Outreach program. Primary responsibilities include providing screening and assessment for Mental Health and Substance Use needs amongst patients accessing services through the Sexual and Behavioral Health (SBH) grant and the provision of short term low threshold mental health and substance use screening sessions and coordinated linkage to care services. Responsibilities will also include internal data collection, reporting, and analyzing for contractual submission. In addition, the SBH Behavioral Health Specialist will participate in Callen-Lorde’s community outreach, education and marketing activities, through community events in the New York metropolitan area, and through collaborations to other CBOs, and will include involvement in the design, implementation and maintenance of multiple prevention interventions unique to Callen-Lorde.

Health Center is open Monday through Saturday. Evening and Saturday hours, along with service delivery in both the Bronx and Manhattan locations are required. SOCIAL WORKER

Position Summary

The Social Worker will provide individual, family/couple and group psychotherapy sessions, supportive counseling, crisis intervention, care coordination, and referral services,. The position will work in collaboration with Callen-Lorde medical staff and consulting psychiatrists, as a member of our multi-disciplinary team. The overall goal of the position is to ensure that patients’ psychosocial needs are addressed and met.

Position Summary

PATIENT CARE ASSOCIATE

Position Summary

We are seeking an exceptionally friendly, organized individual with computer proficiency and experience in medical settings to join our Medical Administrative Services Department as a Patient Care Associate (PCA). The PCA is part of our dynamic medical team and works with collaboration with our nursing and provider staff to ensure optimal patient care. The PCA is responsible for maintaining an efficient flow of patients, visitors, and phone calls into various systems of the Center. The PCA is a primary customer service contact with our patients, therefore, must be courteous, patient, and professional at all times.
