DENTAL ASSISTANT

Position Summary

We are seeking exceptionally friendly, organized individuals with excellent skills to join our staff as Dental Assistants. In this position you will provide clinical support to dentists and hygienists, including preparation of patients, supplies, and rooms; chair side assistance; radiology and laboratory assistance. Additionally, you will be responsible to ensure appropriate and efficient patient flow for the Dental Providers.

Work Schedule/Salary/Benefits

The Dental Clinic is open Monday through Friday, hours of operation and related shifts range from early morning to late evenings. Evenings are required. Qualifications

  • Training and certification as a Dental Assistant.

CLINICAL DIRECTOR OF ADOLESCENT/YOUNG ADULTS PROGRAM (FULL-TIME MEDICAL PROVIDER) – HEALTH OUTREACH TO TEENS (HOTT)

Position Summary

The Clinical Director of HOTT is responsible for overseeing the delivery of comprehensive primary care to our adolescent patients, including HIV+, homeless and questioning youth in a sensitive and non-judgmental manner. He/she is expected to maintain a high degree of professional competence and the highest possible ethical standards, and to function as clinical leader of a care-giving team. Administrative responsibilities include the training, education and clinical supervision of staff providing adolescent care, the development of clinical protocols in conjunction with the Chief Medical Officer, assisting with management of Youth related Grant funded Program requirements, and participation in quality management. He/she will function as part of a team to develop strategic growth initiatives for the HOTT Program.

Work Schedule/Salary/Benefits

The Health Center is open Monday through Saturday. This full time position requires weekly 12 noon – 8:00 pm shifts as well some Saturdays on rotation. Qualifications

  • Current New York State License and Registration as a MD, DO or FNP.

  • DEA Controlled Substances Registration Certificate and BCLS or ACLS certification required.
  • At least 3 years of clinical experience in HIV, adolescent medicine and/or working with lesbian, gay, bisexual and/or transgender populations strongly preferred.

LICENSED PRACTICAL NURSE

Position Summary

The Licensed Practical Nurse (LPN) is responsible for assisting primary care providers — physicians and mid-level providers by providing general nursing care, patient screening, and clinical support. This position reports to the Nursing Supervisor.

MANAGER OF INTEGRATED HEALTH SERVICES

Position Summary

The Manager of Integrated Health Services (MIHS) will ensure efficient administrative operation for the mental health and medical services provided at Callen-Lorde’s Article 31 Clinic. The MIHS will oversee daily operations and staffing and address patient concerns and complaints. The manager will also monitor visit volume, provide budget management and benchmark standards to develop corrective plans to improve service delivery. The MIHS will work in collaboration with the Director of Integrated Health and the Director of Psychiatry to ensure that clinical standards, funder, and regulatory compliance requirements are met. The MIHS will also conduct community outreach and develop partnerships to promote Callen-Lorde services to underserved populations. In addition, the MIHS will ensure the clinic is in compliance with all aspects of Article 31 and Article 28 service delivery documentation and contract deliverables for funding sources.

MEDICAL RECORDS MANAGER

Position Summary

Callen-Lorde is seeking a dedicated individual to join our team as the Medical Records Manager (MRM). This person will manage the day-to-day operations of the Medical Records department, insuring HIPAA-compliant handling of Health Information. The MRM will serve as Callen-Lorde’s Privacy Officer and oversee all privacy matters for the organization. The MRM will ensure the maintenance, integrity and security of the electronic and paper medical records system and coordinate the handling of PHI to ensure it meets New York State’s health information security and compliance standards. This person will be a working supervisor and lead a team responsible for responding to all requests for patient records, monitor telephone calls, lab entries, missing labs, scanning and filing. This person will directly address patient concerns and complaints. The MRM will contribute to the ongoing development, evaluation and improvement of Patient Services within the Medical Records department.

PATIENT CARE ASSOCIATE

Position Summary

We are seeking an exceptionally friendly, organized individual with computer proficiency and experience in medical settings to join our Medical Administrative Services Department as a Patient Care Associate (PCA). The PCA is part of our dynamic medical team and works with collaboration with our nursing and provider staff to ensure optimal patient care. The PCA is responsible for maintaining an efficient flow of patients, visitors, and phone calls into various systems of the Center. The PCA is a primary customer service contact with our patients, therefore, must be courteous, patient, and professional at all times.

SBH Behavioral Health Specialist – LMSW/LMHC/CASAC(Part-time)

Position Summary

The part time SBH Behavioral Health Specialist will participate in all aspects of the agency’s Prevention and Outreach program. Primary responsibilities include providing screening and assessment for Mental Health and Substance Use needs amongst patients accessing services through the Sexual and Behavioral Health (SBH) grant and the provision of short term low threshold mental health and substance use screening sessions and coordinated linkage to care services. Responsibilities will also include internal data collection, reporting, and analyzing for contractual submission. In addition, the SBH Behavioral Health Specialist will participate in Callen-Lorde’s community outreach, education and marketing activities, through community events in the New York metropolitan area, and through collaborations to other CBOs, and will include involvement in the design, implementation and maintenance of multiple prevention interventions unique to Callen-Lorde.

SOCIAL WORKER

Position Summary

The Social Worker will provide individual, family/couple and group psychotherapy sessions, supportive counseling, crisis intervention, care coordination, and referral services,. The position will work in collaboration with Callen-Lorde medical staff and consulting psychiatrists, as a member of our multi-disciplinary team. The overall goal of the position is to ensure that patients’ psychosocial needs are addressed and met.

SOCIAL WORKER – ADOLESCENT/YOUNG ADULTS HEALTH OUTREACH TO TEENS (HOTT) PROGRAM

Position Summary

We are seeking a dedicated, flexible individual to join our staff as a full time Social Worker within our Health Outreach to Teens, (HOTT) program. HOTT provides low- and no-cost medical and mental health care to LGBTQ and homeless youth, ages 13-24. The Social Worker is responsible for providing clinical social work services to engage youth accessing medical and mental health services within the HOTT program. This includes short-term individual counseling, group counseling, family counseling, crisis management, and psycho-educational services for youth. The Social Worker is responsible for appropriate documentation of care, as well as written mental health assessments and treatment plans. The HOTT Social Worker will work closely with medical, case management and outreach workers in a harm reduction model, to ensure that the needs of HOTT clients are identified and appropriate services accessed, coordinated, and monitored.

SUPERVISING HIV CARE COORDINATOR

Position Summary

Callen-Lorde Community Health Center is seeking a dedicated, flexible individual to join our staff as a Supervising Care Coordinator (SCC). The SCC is responsible for supervising four case managers who provide case management in one of three areas: HIV, transgender services, or women’s health services. The SCC will be a working supervisor with their own caseload of patients. The SCC team will assist patients with treatment education and supportive counseling. They will also work closely with medical, mental health and outreach workers to ensure that the full continuum of care needs of our clients are identified and appropriate services accessed, coordinated, and monitored.

TRANSGENDER OUTREACH AND PREVENTION COORDINATOR

Position Summary

The Transgender Prevention Coordinator will participate in all aspects of the agency’s Prevention and Outreach program with a focus on grants and funding specific to young transgender (YTG) people of color. Primary responsibilities include coordinating and providing rapid HIV antibody testing, screening and assessment, risk reduction counseling, confirmatory testing and the coordination of on-site HIV preventions services, including but not limited to CLEAR and Personalized Cognitive Counseling (PCC), for young transgender people of color between 16-29 and their sexual partners. Additional responsibilities include troubleshooting, productivity monitoring and evaluation, case follow-up, coordinating linkage to care services, internal data collection, entry, reporting, and analyzing for contractual submission. Further responsibilities include designing and implementing a YTG of color HIV prevention campaign, managing annual community advisory boards consisting of YTG of color involved in the media campaign design and program intervention assessments, community outreach at House & Ball events and the coordination of an annual YTG of color focused community event. In addition, the MMI Coordinator will participate in Callen-Lorde’s community outreach, education and marketing activities, through community events in the New York metropolitan area, and through collaborations with other CBOs, and will be involved in the design, implementation and maintenance of multiple prevention interventions unique to Callen-Lorde.
