DENTIST

Position Summary

Callen-Lorde is seeking an experienced, dedicated Dentist to join our team in providing oral health services to a diverse patient population, including those living with HIV. In this role you will perform routine preventative and maintenance dentistry, as well as complex treatments for advance oral diseases and injuries of teeth and gums.

Work Schedule/Salary/Benefits

The Health Center is open Monday through Saturday. Weekly evening and rotating Saturday hours are required. HIV & PRIMARY CARE PROVIDER IN ARTICLE 31 MENTAL HEALTH FACILITY

Position Summary

This is an exciting opportunity to work at Callen-Lorde in a new, state of the art Article 31 Mental Health Facility, soon to open in Chelsea. Within the context of multidisciplinary treatment team model, the Provider will serve as the Primary Care Provider for patients also receiving Article 31 mental health treatment and care coordination services at the facility. We are seeking an experienced, dedicated clinician (MD, DO, NP, PA) to join our team in providing primary care services to a panel of diverse patients, many who are HIV+.

Work Schedule/Salary/Benefits

Qualifications

  • New York State Certification/Licensure and current New York State Registration (MD, DO, NP, or PA)

  • Substantial experience in HIV primary care. (Minimum of 2 years) Experience in sexually transmitted infection screening, treatment, and education

INSURANCE NAVIGATOR

Position Summary

Callen-Lorde is seeking an Insurance Navigator, responsible for identifying eligible candidates and assisting with enrollment in Medicaid, Family Health Plus and Child Health Plus as well as assisting current members to re-certify into these plans. The position requires frequent travel through NYC in the boroughs of Manhattan, Queens, Brooklyn and the Bronx. Consistently meeting established monthly enrollment quotas is a job requirement.

MAJOR GIFTS OFFICER

Position Summary

The Major Gifts Officer (MGO) is responsible for the development and successful execution of the health center’s fundraising programs, working closely with the Executive Director and the Board of Directors. Fundraising initiatives include individual giving, major donors, special events, and foundation/corporate giving. The Major Gifts Officer will be expected to play an especially hands-on role in the growth of major donor program and individual giving. The MGO assists the Manager of Communications in the development and delivery of an external communications strategy that increases awareness and understanding of Callen-Lorde’s mission and services to the community at large, the media, and governmental bodies. The Major Gifts Officer will be the lead solicitor for securing major gifts and sponsorships, working with the Manager of Communications and the Special Projects Coordinator to achieve fundraising and communications goals.

This position is scheduled Monday through Friday. Some evening hours will be required.     • Bachelor’s degree in relevant field required. Advanced degree preferred.

MENTAL HEALTH PATIENT NAVIGATOR

Position Summary

Callen-Lorde is seeking a flexible, dedicated individual to join our staff as a Mental Health Patient Navigator. This innovative new position will work closely with the Clinical Coordinator for Mental Health, the Mental Health Services staff, and the department of Care Coordination. The Mental Health Patient Navigator is responsible for the coordination of mental health services including hospital discharge planning, accompaniment to/from mental health appointments, promotion of medication adherence, and assistance with benefits and entitlements. This is a field-based position working with patients in their homes and accompanying them to appointments, as such the schedule is determined by patient needs. The goals of the Mental Health Patient Navigation activities are to ensure patients are linked to mental health services in a timely and coordinated manner and maintain patients in care via navigation, coordination of mental health and social services and provision of support and coaching; additionally, the Mental Health Patient Navigator will assist in engaging patients in primary medical care when necessary. The Mental Health Patient Navigator will assist in doing so by teaching and supporting treatment (medications) adherence techniques to patients.

NEXTGEN EHR SPECIALIST

Position Summary

Callen-Lorde is seeking a flexible, dedicated individual to join our staff as a NextGen EHR Specialist (NES). The NES will develop, customize and test NextGen EHR templates and documents. The NES will troubleshoot issues related to the NextGen EHR and ensure that patient information is organized and maintained appropriately in the system. The NES will also ensure that templates and documents meet all internal and external requirements and regulations. This person will work in Callen-Lorde’s Health Information Technology (HIT) department and report to the Senior Director of Strategic Initiatives.

Work Schedule/Salary/Benefits

Callen-Lorde is open Monday through Saturday. This is a full-time position with normal business hours (9:00AM to 5:00PM Monday – Friday). Occasional evenings and Saturdays will be required. PATIENT CARE ASSOCIATE

Position Summary

We are seeking an exceptionally friendly, organized individual with computer proficiency and experience in medical settings to join our Medical Administrative Services Department as a Patient Care Associate (PCA). The PCA is part of our dynamic medical team and works with collaboration with our nursing and provider staff to ensure optimal patient care. The PCA is responsible for maintaining an efficient flow of patients, visitors, and phone calls into various systems of the Center. The PCA is a primary customer service contact with our patients, therefore, must be courteous, patient, and professional at all times.

PATIENT SUPPORT LIASON – LMSW/LCSW

Position Summary

The Patient Liaison will work with individuals whose psychosocial needs have not permitted successful functioning as patients at our community health center. The liaison’s role is to provide assessment of patients deemed disruptive, (agency referred) and self-referred patients requiring assistance to determine if they are able to avoid a behavioral discharge and be retained in care.This will include ongoing assessments, assisting patients to successfully navigate Callen-Lorde’s systems, developing behavioral goals and creating patient behavioral treatment plans. In this role you will be the person to respond when a patient onsite is experiencing a mental health emergency and will serve as a key member of the Disruptive Patient Committee. The position will be working in collaboration with the Manager of Quality, Risk and Compliance in conjunction with the Patient Advocate, Mental Health providers and Care Coordination staff. The overall goal of the position is to ensure that such vulnerable patients may continue to receive quality care at Callen-Lorde.

REGISTERED NURSE, SCREENING

Position Summary

The Registered Nurse is responsible for assisting primary care providers by screening patient phone calls and patient walk-ins for urgent medical matters, triaging patients to urgent care, and referring patients to the ER or other facilities. The RN will assist providers by providing education, adherence and case management support to patients. Other responsibilities will include following up on calls from the laboratory regarding alert values, providing education, information and support to patients regarding a variety of medical issues, including HIV and HIV adherence, and providing clinical support during patient care sessions when needed.

Position Summary

The Social Worker will provide individual, family/couple and group psychotherapy sessions, supportive counseling, crisis intervention, care coordination, and referral services,. The position will work in collaboration with Callen-Lorde medical staff and consulting psychiatrists, as a member of our multi-disciplinary team.
