ASSOCIATE DIRECTOR OF MEDICINE

Position Summary

The Associate Director of Medicine (ADM) is an exciting newly created position at Callen-Lorde Community Health Center, a dynamic, innovative FQHC serving NYC’s lesbian, gay, bisexual and transgender community. The ADM will join the Clinical leadership team and work in close collaboration with the Director of Medicine in providing clinical and administrative leadership and vision to Callen-Lorde’s medical practice, particularly to the primary care service. Key responsibilities will include mentoring new medical providers, supervising and evaluating half of the 20+ MD/DO/NP/PAs and promoting quality improvement initiatives throughout the Department of Medicine. This ADM will provide input into the development of electronic medical record (EMR) templates as well as the overall functionality and regulatory compliance of the EMR. This position will also provide direct patient care on a part-time basis (five sessions weekly) documenting care promptly and following charting protocol and applicable NYS licensing requirements. There will also be opportunities for teaching, research and faculty appointment.

Work Schedule/Salary/Benefits

Callen-Lorde is open Monday through Saturday. This position requires weekly evening and occasional weekend and on call coverage. Qualifications

  • New York State-Licensed Physician Board Certified in Internal Medicine, Family Practice, and/or Infectious Disease.

  • At least three years of clinical experience in an outpatient medical setting required; Experience providing primary care and HIV/AIDS- related care highly preferred.
  • Prior supervisory experience is preferred but not required.
  • Commitment to community based health care services and the mission of Callen-Lorde.

CHIEF TECHNOLOGY OFFICER

Position Summary

Callen-Lorde is seeking a Chief Technology Officer (CTO) to join our Senior Management team. The CTO will provide direction and supervision to Callen-Lorde’s IT and HIT departments and foster the health clinic’s technological development. Responsibilities will include evaluating existing technology, selecting, budgeting and implementing new technology to support strategic initiatives. The CTO will collaborate with IT management consultants to develop and achieve strategic goals and improve efficiency in Callen-Lorde’s technological systems and programs. The CTO will serve as the health clinic’s liaison to external IT communities, ensuring the center’s ability to participate in varied incentive programs, and be attuned to external developments in funding opportunities, technologies, and best practices.

Qualifications

  • Bachelor’s Degree required (Computer science, business, or equivalent certifications or technical expertise).
  • 5+ years management experience with computer information systems including security, networking, server, software application management and support within a healthcare setting.

CLINICAL INFORMATION REPORTING ANALYST

Position Summary

The goal of the Clinical Information Reporting Analyst position is facilitation of excellent data analysis, reporting and collection in the provision of healthcare services at Callen-Lorde. The Analyst develops data collection tools within the Electronic Health Record (EHR) in conjunction with managers and clinical staff; assists in the implementation of those tools; designs and performs analyses of the data therein, provides guidance to staff in running and interpreting such analyses and other data reports; provides liaison with EHR and other clinical database vendors. The Analyst will also develop and/or assist in the development of database interfaces with external business partners and public health agencies and will provide technical guidance to managers regarding the utilization of information technology in streamlining clinical and business tasks.

Qualifications

COORDINATOR, PATIENT SUPPORT AND QUALITY

Position Summary

The Coordinator of Patient Support & Performance Improvement primary responsibility is to assist the Patient Support Manager with patient feedback collection, resolution and de-escalate disruptive patients during assigned shifts. The Coordinator will also support the Quality Department in collection, analysis and dissemination of quality metric data, as well as supporting Callen Lorde’s performance improvement efforts. Additionally, the Coordinator is responsible for supporting all areas of Callen-Lorde’s Risk Management Plan, Quality Management Plan, Safety Program, and Corporate Compliance Program.

DIRECTOR OF HEALTH OUTREACH TO TEENS (HOTT) SERVICES

Position Summary

Callen-Lorde Community Health Center seeks a dedicated, flexible individual to join our management team as the Director of Health Outreach to Teens (HOTT). The HOTT Program provides no– and low-cost medical and mental health care to LGBTQ youth, ages 13-24. The Director is responsible for the administrative operations, program development, government and private grants management, quality management, and marketing/outreach. To this end, the Director oversees program services delivery, both on-site and on the mobile medical unit; leads the staff in the ongoing development, evaluation, and improvement of the program; coordinates quality assurance efforts in conjunction with clinical staff and other Callen-Lorde managers; coordinates outreach efforts to expand knowledge of HOTT for both the youth services community and communities of young people; and, prospecting for program funding, as well as management of funding contracts.

Work Schedule/Salary/Benefits

Health Center is open Monday through Saturday; the HOTT program does not currently operate on Saturdays. Evening hours are required. DIRECTOR, INTEGRATED MENTAL HEALTH SERVICES

Position Summary

Callen-Lorde has opened a brand new site for integrated Mental Health and Medical Services in Chelsea serving the city’s lesbian, gay, bisexual and transgender population. The Director OF Integrated Mental Health Services (DIMHS) will oversee the clinical delivery of efficient, high quality psychotherapy and recovery services to our patients The DIMHS will work in collaboration with the Manager of Integrated Health and the Director of Psychiatry to, as a management team, ensure that clinical standards, funder, and regulatory compliance requirements are met and provide oversight & supervision of the clinical team. In addition, the DMIHS will ensure the clinic is in compliance with all aspects of Article 31 and Article 28 service delivery documentation and contract deliverables for funding sources. The DIMS will provide clinical oversight to all operations and as part of the management team, also monitor visit volume, provide budget management and benchmark standards to develop corrective plans to improve service delivery.

Work Schedule/Salary/Benefits

Callen-Lorde’s 17th Street clinic will be open Monday through Friday. This is a full-time position and will include some weekly evening hours. GYNECOLOGIST – PART-TIME

Position Summary

Callen-Lorde is seeking an experienced, dedicated, part-time gynecology clinician (MD, DO) to join our team in providing gynecological care to a diverse patient population, including adults and adolescents living with HIV. This provider will perform annual testing and exams, diagnose of illnesses, pap test procedures, gynecologic colposcopies, as well as High Resolutions Anoscopy (HRA). In addition, you will participate in our newly established alternative insemination program.

HEALTH HOMES PATIENT NAVIGATOR

Position Summary

Callen-Lorde is seeking a dedicated, flexible individual to join our staff as a Health Homes Patient Navigator (Navigator). The Navigator will work closely with the department of Care Coordination, Medical Providers and Mental Health Providers. The Navigator is responsible for the coordination of care for patients who are enrolled in Medicaid and are living with chronic illnesses. Responsibilities include hospital discharge planning, accompaniment to/from medical and mental health appointments, promotion of medication adherence, health education and assistance with benefits and entitlements. This is a field-based position working with patients in their homes and accompanying them to appointments. The goals of this position is to ensure patients are linked to healthcare services in a timely and coordinated manner and maintain patients in care via navigation, coordination of primary medical care and social services and provision of support and coaching.

NEXTGEN SUPPORT ANALYST

Position Summary

Callen-Lorde is seeking a flexible, dedicated individual to join our staff as a NextGen Support Analyst (SA). The SA will serve as a technical resource for the health center’s NextGen Practice Management (PM), Electronic Health Record (EHR), Electronic Dental Record (EDR) and Document Management (formerly ICS) systems. The SA will work within the Health Information Technology (HIT) team to provide end user support. In this role the SA will track all HIT related support requests assuring that an appropriate member of the team is following up on any issues in a timely manner, this will include monitoring all support tickets opened with NextGen to assure their timely response and escalation of issues as necessary. In this role the SA will identify and troubleshoot issues within NextGen systems, working to resolve them by identifying existing NextGen resources and independent investigation, the SA will perform all follow-up activities to assure satisfactory resolution of issues and communication with the end users. The SA will participate in the implementation of all system software enhancements and upgrades. This position may also include EHR template and document development as well report development in EHR and PM systems.

SUPERVISOR, RESEARCH STUDIES (For SPARK PrEP Demonstration Project)

Position Summary

Callen-Lorde Community Health Center (CLCHC) and the Hunter HIV/AIDS Research Team (HART) are seeking a full-time site Research Study Supervisor (SRS) for SPARK, an NIH-funded PrEP Demonstration Project and research study. The study is funded to develop and test an intervention designed to integrate HIV pre-exposure prophylaxis (PrEP) delivery and support into a comprehensive HIV prevention package delivered at a community health center (Callen Lorde). We are looking for an exceptional and dedicated individual with strong organizational skills, clinical research experience, and creative problem-solving abilities. This person will supervise study operations and staff at CLCHC, and will work closely with the collaborating PI (Dr. Sarit Golub) and her research team at HART/Hunter College, CUNY and the Senior Director of Research and Education at CLCHC. Opportunities for professional development and engagement in HIV prevention policy are also available.

LICENSED PRACTICAL NURSE

Position Summary

The Licensed Practical Nurse (LPN) is responsible for assisting primary care providers — physicians and mid-level providers by providing general nursing care, patient screening, and clinical support. This position reports to the Nursing Supervisor.

Work Schedule/Salary/Benefits

The Health Center is open Monday through Saturday. Two evening shifts per week are required. Salary will depend on experience and will be accompanied by an excellent benefits package, including individual medical and dental insurance, life insurance, short- and long-term disability insurance, generous vacation/leave time, and optional 401K/403B plans.

Qualifications

  • New York State LPN License.
  • Current CPR certification.
  • Strong Computer Competency.

PATIENT CARE ASSOCIATE

Position Summary

We are seeking an exceptionally friendly, organized individual with computer proficiency and experience in medical settings to join our Medical Administrative Services Department as a Patient Care Associate (PCA). The PCA is part of our dynamic medical team and works with collaboration with our nursing and provider staff to ensure optimal patient care. The PCA is responsible for maintaining an efficient flow of patients, visitors, and phone calls into various systems of the Center. The PCA is a primary customer service contact with our patients, therefore, must be courteous, patient, and professional at all times.

Work Schedule/Salary/Benefits

Full time position – Minimum of 35 hours per week. Salary is paid at an hourly rate, plus excellent benefits package.

Qualifications

      • BS/BA degree in Social Service related discipline.
      • Strong computer competency (MS Office); ability to learn new software.
      • Bilingual (English/Spanish) strongly preferred.
      • Strong customer service orientation and excellent attention to detail.
      • Commitment to the mission of Callen-Lorde.
      • Scheduling flexibility and the ability to handle multiple simultaneous tasks is a must.
      • Ability to remain calm, focused, and helpful under stress while working with diverse populations.
