ASSOCIATE DIRECTOR, MEDICINE

Position Summary

The Associate Director of Medicine (ADM) collaborates with the Director of Medicine in providing clinical and administrative leadership and vision to Callen-Lorde’s medical practice, particularly to the primary care service. The ADM assists the Director of Medicine in managing the professional, organizational, and administrative aspects of the quality of care provided to Callen-Lorde patients. In addition this role will provide direct supervision to a group of Medical Providers and support the orientation and onboarding of all new Medical Providers. This role will also provide input into the development of electronic medical record templates as well as the overall functionality and regulatory compliance of the EMR. This position will also provide direct quality, sensitive patient care on a part-time basis (five sessions weekly) documenting care promptly and following charting protocol and applicable NYS licensing requirements. The ADM will maintain a small panel of primary care patients, provide coverage when other Medical Providers are out of the office as well as working in the Urgent Care and Sexual Health Programs.

Work Schedule/Salary/Benefits

The Health Center is open Monday through Saturday. This position requires weekly evening and occasional weekend and on call coverage. Qualifications

  • York State-Licensed Physician Board Certified in Internal Medicine, Family Practice.
  • Certification as an HIV Specialist highly preferred.
  • At least three years of clinical experience in an outpatient medical setting required; Experience providing primary care and HIV/AIDS- related care is required.
  • Prior supervisory experience is preferred but not required.

GYNECOLOGIST – PART-TIME

Position Summary

Callen-Lorde is seeking an experienced, dedicated, gynecologist to join our team on a part-time basis. This role will provide gynecological care to a diverse female bodied patient population including patients who identify as lesbian, bisexual, heterosexual, gender non-conforming, and transgender. The ideal candidate will provide routine and urgent GYN care, perform colposcopies and high resolutions anoscopies (HRA), as well as participate in and expand our alternative insemination program. This candidate will be the collaborating physician for mid-level women’s health providers.

Work Schedule/Salary/Benefits

The Health Center is open Monday through Saturday day and evening. We are seeking someone who can commit to working approximately a 15 hours per week schedule as a per diem.

Qualifications

  • New York State Certification/Licensure and current New York State Registration.
  • Board Certification in Gynecology.

HOTT – TRANSGENDER/GENDER NON-CONFORMING (T/GNC) MEDICAL CASE MANAGER

Position Summary

Callen-Lorde is seeking a dedicated, flexible individual to join our staff as a Youth (T/GNC) Medical Case Manager within our Health Outreach to Teens, or HOTT program. The HOTT Program provides low- and no-cost medical and mental health care to LGBTQ and homeless youth, ages 13-24. The Medical Case Manager (MCM) provides a range of patient-centered services that coordinates all medical related services, diminishes barriers to care and facilitates the receipt of social and supportive services to achieve optimal health. The coordination and follow-up of medical appointments are key activities of MCM. These services ensure timely and coordinated access to health and support services, through ongoing assessment of patient needs and personal support systems. This includes the provision of treatment adherence counseling to ensure adherence to medications, as well as self-management skill development and health education. In addition, the T/GNC MCM will ensures medical case management services are available to the patient and are documented in the patient’s record, develop an initial psychosocial assessment of service needs of patient is conducted, identifies the needs, resources and strengths of the patient that form the patient’s individualized service plan.

Qualifications

  • MSW preferred, college degree (BA/BS) in health/social services or the equivalent.
  • Experience in medical/social services (case management) and or entitlements work preferred.

  • Strong computer competency, including knowledge of Electronic Medical Records, Computerized Appointment/Billing Programs and other Microsoft Office Programs.

INSURANCE NAVIGATOR

Position Summary

The Insurance Navigator is responsible for identifying eligible candidates and assisting with enrollment in Medicaid, Family Health Plus and Child Health Plus as well as assisting current members to recertify into these plans. The position requires frequent travel through NYC in the boroughs of Manhattan, Queens, Brooklyn and the Bronx.Consistently meeting established monthly enrollment quotas is a job requirement.

NEXTGEN SUPPORT ANALYST

Position Summary

Callen-Lorde is seeking a flexible, dedicated individual to join our staff as a NextGen Support Analyst (SA). The SA will serve as a technical resource for the health center’s NextGen Practice Management (PM), Electronic Health Record (EHR), Electronic Dental Record (EDR) and Document Management (formerly ICS) systems. The SA will work within the Health Information Technology (HIT) team to provide end user support. In this role the SA will track all HIT related support requests assuring that an appropriate member of the team is following up on any issues in a timely manner, this will include monitoring all support tickets opened with NextGen to assure their timely response and escalation of issues as necessary. In this role the SA will identify and troubleshoot issues within NextGen systems, working to resolve them by identifying existing NextGen resources and independent investigation, the SA will perform all follow-up activities to assure satisfactory resolution of issues and communication with the end users. The SA will participate in the implementation of all system software enhancements and upgrades. This position may also include EHR template and document development as well report development in EHR and PM systems.

Callen-Lorde is open Monday through Saturday. This is a full-time position with normal business hours (9:00AM to 5:00PM Monday – Friday). Occasional evenings and Saturdays will be required. PREP PROGRAM SUPERVISOR

Position Summary

The PrEP (Pre-exposure Prophylaxis) Program Supervisor will supervising two PrEP Specialists and oversee the delivery of PrEP services to all patients enrolled in the PrEP Program at Callen-Lorde. PrEP services include eligibility assessments, PrEP access navigation, insurance and/or Medication Assistance Program (MAP) linkage and assistance. Services also include HIV and HCV testing and linkage to care, STI screenings and treatment, Hep A & B screenings and vaccination, and linkage to nPEP. The PrEP Program Supervisor will also develop and maintain PrEP related protocols and guidelines, and perform contract reporting, data monitoring and entry. The Supervisor will establish and maintain relationships with external service providers, troubleshoot service delivery issues, monitor productivity and assist with the service delivery of government funded HIV, HCV and STI Testing and Prevention services. In addition, the PrEP Program Supervisor will also participate in community outreach, education and marketing events and collaborate with other CBOs.

SBH Behavioral Health Specialist – LMSW/LMHC/CASAC(Part-time)

Position Summary

The part time SBH Behavioral Health Specialist will participate in all aspects of the agency’s Prevention and Outreach program. Primary responsibilities include providing screening and assessment for Mental Health and Substance Use needs amongst patients accessing services through the Sexual and Behavioral Health (SBH) grant and the provision of short term low threshold mental health and substance use screening sessions and coordinated linkage to care services. Responsibilities will also include internal data collection, reporting, and analyzing for contractual submission. In addition, the SBH Behavioral Health Specialist will participate in Callen-Lorde’s community outreach, education and marketing activities, through community events in the New York metropolitan area, and through collaborations to other CBOs, and will include involvement in the design, implementation and maintenance of multiple prevention interventions unique to Callen-Lorde.

YOUTH HIV MEDICAL CASE MANAGER (HOTT Program)

Position Summary

The Callen-Lorde Community Health Center seeks a dedicated, flexible individual to join our staff as a Youth Medical Case Manager within our Health Outreach to Teens, or HOTT program. The HOTT Program provides low- and no-cost medical and mental health care to LGBTQ and homeless youth, ages 13-21. Youth services are provided both on-site and on our mobile medical unit. The Medical Case Manager is responsible for providing medical case management and supportive services to HIV positive youth accessing medical services within the HOTT program. This includes registration/data entry, financial screening, entitlements enrollment and referrals for youth accessing care at HOTT. The HOTT Medical Case Manger will work closely with primary medical care providers, adherence nurses, mental health and outreach workers to ensure that the bio-psychosocial continuum of care needs of HOTT clients are identified and appropriate services accessed, coordinated, and monitored. The Medical Case Manager plays a critical role in ensuring that HIV positive patients achieve adherence to primary care recommendations and successfully transition to adult systems of care.

PATIENT CARE ASSOCIATE

Position Summary

We are seeking an exceptionally friendly, organized individual with computer proficiency and experience in medical settings to join our Medical Administrative Services Department as a Patient Care Associate (PCA). The PCA is part of our dynamic medical team and works with collaboration with our nursing and provider staff to ensure optimal patient care. The PCA is responsible for maintaining an efficient flow of patients, visitors, and phone calls into various systems of the Center. The PCA is a primary customer service contact with our patients, therefore, must be courteous, patient, and professional at all times.
