HEPATITIS TLC COORDINATOR

Position Summary

The Hepatitis TLC Coordinator (HTC) will participate in all aspects of the agency’s Prevention and Outreach program. Primary responsibilities include coordinating and providing rapid HIV and Hep C antibody testing, screening and assessment, risk reduction counseling, and confirmatory testing to clients at off-site venues and partner organizations, and the Coordination of on-site Hep C testing, troubleshooting, and productivity monitoring and evaluation in accordance with established Callen-Lorde testing protocols. Coordination with community partner sites will also require relationship building, site searching, and productivity monitoring and evaluation. Responsibilities will include phlebotomy venipuncture blood draws, case follow-up and coordinated linkage to care services. Responsibilities will also include internal data collection, entry, reporting, and analyzing for contractual submission. In addition, the HTC will participate in Callen-Lorde’s community outreach, education and marketing activities, through community events in the New York metropolitan area, and through collaborations with other CBOs, and will be involved in the design, implementation and maintenance of multiple prevention interventions unique to Callen-Lorde.

Health Center is open Monday through Saturday. Evening and Saturday hours are required. Qualifications

  • College degree (BA/BS) in health/social services required.
  • Experience in health care, HIV service delivery or social services preferred.

LICENSED PRACTICAL NURSE

Position Summary

The Licensed Practical Nurse (LPN) is responsible for assisting primary care providers — physicians and mid-level providers by providing general nursing care, patient screening, and clinical support. This position reports to the Nursing Supervisor.

MAJOR GIFTS OFFICER

Position Summary

The Major Gifts Officer (MGO) is responsible for the development and successful execution of the health center’s fundraising programs, working closely with the Executive Director and the Board of Directors. Fundraising initiatives include individual giving, major donors, special events, and foundation/corporate giving. The Major Gifts Officer will be expected to play an especially hands-on role in the growth of major donor program and individual giving. The MGO assists the Manager of Communications in the development and delivery of an external communications strategy that increases awareness and understanding of Callen-Lorde’s mission and services to the community at large, the media, and governmental bodies. The Major Gifts Officer will be the lead solicitor for securing major gifts and sponsorships, working with the Manager of Communications and the Special Projects Coordinator to achieve fundraising and communications goals.

Work Schedule/Salary/Benefits

Qualifications

    • Bachelor’s degree in relevant field required. Advanced degree preferred.

    • Familiarity with New York’s philanthropic LGBT community.

MEDICAL RECORDS MANAGER

Position Summary

Callen-Lorde is seeking a dedicated individual to join our team as the Medical Records Manager (MRM). This person will manage the day-to-day operations of the Medical Records department, insuring HIPAA-compliant handling of Health Information. The MRM will serve as Callen-Lorde’s Privacy Officer and oversee all privacy matters for the organization. The MRM will ensure the maintenance, integrity and security of the electronic and paper medical records system and coordinate the handling of PHI to ensure it meets New York State’s health information security and compliance standards. This person will be a working supervisor and lead a team responsible for responding to all requests for patient records, monitor telephone calls, lab entries, missing labs, scanning and filing. This person will directly address patient concerns and complaints. The MRM will contribute to the ongoing development, evaluation and improvement of Patient Services within the Medical Records department.

NEXTGEN EHR SPECIALIST

Position Summary

Callen-Lorde is seeking a flexible, dedicated individual to join our staff as a NextGen EHR Specialist (NES). The NES will develop, customize and test NextGen EHR templates and documents. The NES will troubleshoot issues related to the NextGen EHR and ensure that patient information is organized and maintained appropriately in the system. The NES will also ensure that templates and documents meet all internal and external requirements and regulations. This person will work in Callen-Lorde’s Health Information Technology (HIT) department and report to the Senior Director of Strategic Initiatives.

Callen-Lorde is open Monday through Saturday. This is a full-time position with normal business hours (9:00AM to 5:00PM Monday – Friday). Occasional evenings and Saturdays will be required. PATIENT CARE ASSOCIATE

Position Summary

We are seeking an exceptionally friendly, organized individual with computer proficiency and experience in medical settings to join our Medical Administrative Services Department as a Patient Care Associate (PCA). The PCA is part of our dynamic medical team and works with collaboration with our nursing and provider staff to ensure optimal patient care. The PCA is responsible for maintaining an efficient flow of patients, visitors, and phone calls into various systems of the Center. The PCA is a primary customer service contact with our patients, therefore, must be courteous, patient, and professional at all times.

PREVENTION AND OUTREACH PROVIDER

Position Summary

The Prevention and Outreach Provider (POP) will participate in all aspects of the agency’s Prevention and Outreach program, with the majority of their efforts geared towards working on an NIH-funded study focused on HIV pre-exposure prophylaxis (PrEP). The study (called SPARK) delivers and tests a program that integrates PrEP delivery and support into a comprehensive HIV prevention package delivered at Callen-Lorde. Responsibilities include providing HIV testing, and screening and assessment/risk reduction counseling to patients in both stand alone visits and the Sexual Health Clinic, in accordance with established Callen-Lorde testing protocol. Responsibilities will also include internal data collection, reporting, and analyzing for contractual submission. In addition, the POP will participate in Callen-Lorde’s community outreach, education and marketing activities, through community events in the New York metropolitan area, and through collaborations to other CBOs, and will include involvement in the design, implementation and maintenance of multiple prevention interventions unique to Callen-Lorde. This position will involve working closely with the collaborating Principal Investigator and her research team at Hunter College.

REGISTERED NURSE, SCREENING (BILINGUAL ENGLISH/SPANISH REQUIRED)

Position Summary

The Registered Nurse is responsible for assisting primary care providers by screening patient phone calls and patient walk-ins for urgent medical matters, triaging patients to urgent care, and referring patients to the ER or other facilities. The RN will assist providers by providing education, adherence and case management support to patients. Other responsibilities will include following up on calls from the laboratory regarding alert values, providing education, information and support to patients regarding a variety of medical issues, including HIV and HIV adherence, and providing clinical support during patient care sessions when needed.

SUPPORTIVE HIV CARE COORDINATOR

Position Summary

Callen-Lorde Community Health Center is seeking a dedicated, flexible individual to join our staff as a Supportive HIV Care Coordinator (SCC). The SCC is responsible for providing case management, treatment education and supportive counseling services to HIV-positive individuals accessing medical and mental health services. The SCC will work closely with medical, mental health and outreach workers to ensure that the full continuum of care needs of our clients are identified and appropriate services accessed, coordinated, and monitored.

SOCIAL WORKER

Position Summary

The Social Worker will provide individual, family/couple and group psychotherapy sessions, supportive counseling, crisis intervention, care coordination, and referral services,. The position will work in collaboration with Callen-Lorde medical staff and consulting psychiatrists, as a member of our multi-disciplinary team. The overall goal of the position is to ensure that patients’ psychosocial needs are addressed and met.

TRANSGENDER MEDICAL CASE MANAGER

Position Summary

The Callen-Lorde Community Health Center seeks a dedicated, flexible individual to join our staff as a Medical Case Manager within our Health Outreach to Teens, or HOTT program. The HOTT Program provides low- and no-cost medical and mental health care to LGBTQ and homeless youth, ages 13-24. Youth services are provided both on-site and on our mobile medical unit. The Medical Case Manager is responsible for providing medical case management and supportive services to at risk youth, young adults and transgender patients accessing medical services within the HOTT program. This includes registration/data entry, financial screening, entitlements enrollment and referrals for youth accessing care at HOTT. In addition, this individual will provide family planning services and health education to a variety of HOTT patients (youth, young adults and transgender). The HOTT Medical Case Manger will work closely with primary medical care providers, adherence nurses, mental health and outreach workers to ensure that the bio-psychosocial continuum of care needs of HOTT clients are identified and appropriate services accessed, coordinated, and monitored. The Medical Case Manager plays a critical role in ensuring that patients achieve adherence to primary care recommendations and successfully transition to adult systems of care.
