ASSOCIATE DIRECTOR OF MEDICINE

Position Summary

The Associate Director of Medicine (ADM) is an exciting newly created position at Callen-Lorde Community Health Center, a dynamic, innovative FQHC serving NYC’s lesbian, gay, bisexual and transgender community. The ADM will join the Clinical leadership team and work in close collaboration with the Director of Medicine in providing clinical and administrative leadership and vision to Callen-Lorde’s medical practice, particularly to the primary care service. Key responsibilities will include mentoring new medical providers, supervising and evaluating half of the 20+ MD/DO/NP/PAs and promoting quality improvement initiatives throughout the Department of Medicine. This ADM will provide input into the development of electronic medical record (EMR) templates as well as the overall functionality and regulatory compliance of the EMR. This position will also provide direct patient care on a part-time basis (five sessions weekly) documenting care promptly and following charting protocol and applicable NYS licensing requirements. There will also be opportunities for teaching, research and faculty appointment.

Qualifications

        • Prior supervisory experience is preferred but not required.

CHIEF TECHNOLOGY OFFICER

Position Summary

Callen-Lorde is seeking a Chief Technology Officer (CTO) to join our Senior Management team. The CTO will provide direction and supervision to Callen-Lorde’s IT and HIT departments and foster the health clinic’s technological development. Responsibilities will include evaluating existing technology, selecting, budgeting and implementing new technology to support strategic initiatives. The CTO will collaborate with IT management consultants to develop and achieve strategic goals and improve efficiency in Callen-Lorde’s technological systems and programs. The CTO will serve as the health clinic’s liaison to external IT communities, ensuring the center’s ability to participate in varied incentive programs, and be attuned to external developments in funding opportunities, technologies, and best practices.

Qualifications

          • Bachelor’s Degree required (Computer science, business, or equivalent certifications or technical expertise).
          • 5+ years management experience with computer information systems including security, networking, server, software application management and support within a healthcare setting.
          • Demonstrated leadership in IT and knowledge of leading technology and service approaches to both internal and external delivery in a health care setting.
          • Experience with Federally Qualified Health Centers, NYS Article 28 and NYS Article 31 clinics preferred.

DATA PROGRAM ASSISTANT

Position Summary

Callen-Lorde seeks a flexible, dedicated individual to join our staff as a Data Program Assistant (DPA). The DPA will be responsible for entering all Prevention and Outreach and Health Outreach to Teen department data into reporting systems and internal trackers, and assuring the quality and accuracy of this data. The DPA will enter all grant related services data into the AIDS Institute reporting system (AIRS), the Center for Disease Control reporting system Evaluation Web and the New York City DOHMH reporting system (eSHARE). In addition, the DPA will assist in data collection and reporting as needed and participate in data mining tasks. The position will play an important role in assuring grant service targets are being met and documentation is correct and up-to-date. The DPA will also assist in ensuring that patients are being maintained in treatment, care and services as per grant protocols. Further responsibilities will include other administrative support functions determined by the Prevention and Outreach and HOTT Management team.

DIRECTOR, INTEGRATED MENTAL HEALTH SERVICES

Position Summary

Callen-Lorde has opened a brand new site for integrated Mental Health and Medical Services in Chelsea serving the city’s lesbian, gay, bisexual and transgender population. The Director OF Integrated Mental Health Services (DIMHS) will oversee the clinical delivery of efficient, high quality psychotherapy and recovery services to our patients The DIMHS will work in collaboration with the Manager of Integrated Health and the Director of Psychiatry to, as a management team, ensure that clinical standards, funder, and regulatory compliance requirements are met and provide oversight & supervision of the clinical team. In addition, the DMIHS will ensure the clinic is in compliance with all aspects of Article 31 and Article 28 service delivery documentation and contract deliverables for funding sources. The DIMS will provide clinical oversight to all operations and as part of the management team, also monitor visit volume, provide budget management and benchmark standards to develop corrective plans to improve service delivery.

Work Schedule/Salary/Benefits

Callen-Lorde’s 17th Street clinic will be open Monday through Friday. This is a full-time position and will include some weekly evening hours. Qualifications

          • Master of Social Work (MSW) with Post-masters clinical training preferred or Ph.D. in Psychology.

DIRECTOR, OF PREVENTION AND OUTREACH

Position Summary

Callen-Lorde Community Health Center seeks a dedicated, flexible individual to join our management team as the Director of Prevention and Outreach (Director). The Director’s key areas of responsibility are administrative management of CLCHC’s Prevention and Outreach Department including Prevention and Outreach staff, HIV/HCV/STI counseling and testing, integration and implementation of biomedical interventions (nPEP, PrEP), counseling, behavioral health/sexual health interventions, research projects, community outreach, prevention related programs, events and activities, management of HIV prevention programs, deliverables and reporting and budget monitoring of all Prevention and Outreach contracts and grants. This includes collaborating with colleagues to develop programmatic goals, oversee implementation and manage evaluation plans related to the provision of these services. In addition, participation in the CLCHC management and operations structure to facilitate, promote and assure appropriate access and growth of all Prevention and Outreach related services is required. The Director of Prevention and Outreach will continue to explore and pursue the continued expansion of CLCHC’s Prevention and Outreach services and contracts portfolio.

GYNECOLOGIST – PART-TIME

Position Summary

Callen-Lorde is seeking an experienced, dedicated, part-time gynecology clinician (MD, DO) to join our team in providing gynecological care to a diverse patient population, including adults and adolescents living with HIV. This provider will perform annual testing and exams, diagnose of illnesses, pap test procedures, gynecologic colposcopies, as well as High Resolutions Anoscopy (HRA). In addition, you will participate in our newly established alternative insemination program.

NEXTGEN SUPPORT ANALYST

Position Summary

Callen-Lorde is seeking a flexible, dedicated individual to join our staff as a NextGen Support Analyst (SA). The SA will serve as a technical resource for the health center’s NextGen Practice Management (PM), Electronic Health Record (EHR), Electronic Dental Record (EDR) and Document Management (formerly ICS) systems. The SA will work within the Health Information Technology (HIT) team to provide end user support. In this role the SA will track all HIT related support requests assuring that an appropriate member of the team is following up on any issues in a timely manner, this will include monitoring all support tickets opened with NextGen to assure their timely response and escalation of issues as necessary. In this role the SA will identify and troubleshoot issues within NextGen systems, working to resolve them by identifying existing NextGen resources and independent investigation, the SA will perform all follow-up activities to assure satisfactory resolution of issues and communication with the end users. The SA will participate in the implementation of all system software enhancements and upgrades. This position may also include EHR template and document development as well report development in EHR and PM systems.

PATIENT SUPPORT MANAGER

Position Summary

The Patient Support Manager (PSM) will work with individuals who have encountered difficulty in receiving services as patients at our community health center. The overall goal of the position is to ensure that all patients, including those with challenging behaviors may continue to receive quality care at Callen-Lorde. In this role you will assist patients to successfully navigate Callen-Lorde’s systems and respond to patient feedback and concerns. You will be responsible for feedback collection, resolution and the de-escalation of disruptive patients. Additionally you will be responsible for monitoring service levels in all areas of the health center, making recommendations for service improvements and removing barriers to care. The PSM is responsible for the conducting ongoing annual patient surveys, other service improvement initiatives, and will serve as the agencies Safety Officer. You will lead the Patient Support Committee in identifying and remedying patterns of patient dissatisfaction.

PREVENTION OUTREACH PROVIDER

Position Summary

The Prevention and Outreach Provider (POP) will participate in all aspects of the agency’s Prevention and Outreach program. Responsibilities include providing HIV/HCV/STI testing, linkage to nPEP and PrEP, and screening and assessment/risk reduction counseling to patients in both stand alone visits and the Sexual Health Clinic, in accordance with the established Callen-Lorde testing protocol, and providing counseling related interventions through related funded grants and contracts including but not limited to CLEAR, and Personalized Cognitive Counseling (PCC). Responsibilities will also include internal data collection, reporting, and analyzing for contractual submission. In addition, the POP will participate in Callen-Lorde’s active recruitment and referrals for related prevention programs, research and services, community outreach, education and marketing activities, through community events in the New York metropolitan area and collaborations with other CBOs. Responsibilities will also include involvement in the design, implementation and maintenance of multiple prevention interventions unique to Callen-Lorde. This position will provide services to all Callen-Lorde patients with a focus on working with young transgender individuals of color and their partners (with special interest in fostering a strong presence within the House and Ball community), working with relevant research studies and the application of biomedical interventions within an integrated health care setting.

PSYCHIATRIST/PSYCHIATRIC NURSE PRACTITIONER

Position Summary

Callen-Lorde is seeking an experienced, dedicated, full-time psychiatrist (MD/DO) or a Psych/MH Nurse Practitioner to serve in our integrated mental health and medical clinic. This position requires demonstrated competence in meeting the mental health needs for the LGBT community and persons living with HIV and compliance with NYS OMH regulations. This position will provide psychiatric assessment and psychopharmacological treatment of patients with acute and chronic psychiatric illnesses. You will become part of an integrated team providing mental health and medical services to a wide array of patients.

SQL REPORT WRITER

Position Summary

The SQL Report Writer performs standard and ad-hoc reporting and data analysis for the provision of healthcare services at Callen-Lorde. Utilizing specialized reporting tools, this individual will develop reports with managers and clinical staff for internal and external clients targeting data in the Electronic Health Record (EHR) database and other sources. This individual will also propose new reports as needed, and will provide guidance to staff in running and interpreting such reports. The SQL Report Writer will also develop and/or assist in the development of database interfaces with external business partners and public health agencies and will provide technical guidance to managers regarding the utilization of information technology in streamlining clinical and business tasks.

Work Schedule/Salary/Benefits

Callen-Lorde is open Monday through Saturday. This is a full-time position with normal business hours (9:00AM to 5:00PM, Monday – Friday). Occasional early mornings, evenings and weekends will be required. Qualifications

                • Working knowledge of SQL required.
                • Willingness to process data quality issues for consistent reporting, design and lead reporting projects involving end users.
                • 2-5 years of experience with report development tools (SQL Server Reporting Services (SSRS) 2008 R2 or newer or Crystal Reports XI or newer).
                • Experience working in the healthcare industry preferred, especially EHR/EMR and practice management data analysis, clinical databases, diagnostic coding, and relevant data privacy laws.
                • Design and development experience using Microsoft’s Business intelligence stack preferred (SQL Server Integration Services (SSIS), SQL Server Analysis Services (SSAS), SharePoint reporting/dashboards, PowerView, and Excel PowerPivot.)
                • Excellent business writing and technical documentation skills.
                • Strong oral communication skills.
                • Bachelor’s degree – Information Systems or related field preferred.
                • Sensitivity to LGBT, HIV, and diverse communities required.
                • Ability to handle multiple, simultaneous tasks, and excellent attention to detail.

PATIENT CARE ASSOCIATE

Position Summary

Callen-Lorde is seeking exceptionally friendly and detail oriented individuals to join our staff as a Patient Care Associates (PCA). PCAs are a part of our dynamic medical team and work in collaboration with our nursing and provider staff to ensure optimal patient care. The PCA is responsible for providing front desk reception and maintaining an efficient flow of patients, visitors, and phone calls into the various systems of Callen-Lorde. PCAs providing administrative support to the clinical staff as well as assisting with the collection of patient registration, billing and payment data. The PCA is a primary customer service contact with our patients, therefore, must be courteous, patient and professional at all times.

Callen-Lorde is open Monday through Saturday. Two evening shifts per week and one Saturday per month are required.
