DENTAL ASSISTANT

Position Summary

We are seeking exceptionally friendly, organized individuals with excellent skills to join our staff as Dental Assistants. In this position you will provide clinical support to dentists and hygienists, including preparation of patients, supplies, and rooms; chair side assistance; radiology and laboratory assistance. Additionally, you will be responsible to ensure appropriate and efficient patient flow for the Dental Providers.

Work Schedule/Salary/Benefits

The Health Center is open Monday through Fridays, hours of operation and related shifts range from early morning to late evenings. Evenings are required. Qualifications

FINANCE ASSOCIATE – PAYROLL/ACCOUNTS PAYABLE

Position Summary

The Finance Associate will be responsible for processing accurately employee data (new hires, changes, additional earnings) into the payroll system and processing a biweekly payroll; Additional responsibilities include the review and importing employee time & attendance data from the time system to payroll and reconciling variances utilizing tools in excel. You will create ad-hoc reports and process benefit deductions for 403b plan and other benefit programs. This position will process cash receipts and maintain a monthly cash receipts log which reconciles with all bank deposits. The Finance Associate will support the work of the Senior Accountant in vouchering for grant contracts, including time and effort attestations. The Finance Associate will perform budget modifications under the supervision of the Senior Accountant. The Finance Associate support the Controller in establishing and maintaining procurement contracts, and will periodically review contracts against other opportunities to ensure competitive pricing is obtained for goods and services. In addition, this employee will backup accounts payable duties including, coding of invoices to cost centers and funding sources, invoice accuracy, and check preparation.

The position is scheduled to work Monday through Friday, occasionally including evening hours. Qualifications

  • Bachelor’s Degree required Finance or Accounting major preferred.

LICENSED PRACTICAL NURSE

Position Summary

MAJOR GIFTS OFFICER

Position Summary

The Major Gifts Officer (MGO) is responsible for the development and successful execution of the health center’s fundraising programs, working closely with the Executive Director and the Board of Directors. Fundraising initiatives include individual giving, major donors, special events, and foundation/corporate giving. The Major Gifts Officer will be expected to play an especially hands-on role in the growth of major donor program and individual giving. The MGO assists the Manager of Communications in the development and delivery of an external communications strategy that increases awareness and understanding of Callen-Lorde’s mission and services to the community at large, the media, and governmental bodies. The Major Gifts Officer will be the lead solicitor for securing major gifts and sponsorships, working with the Manager of Communications and the Special Projects Coordinator to achieve fundraising and communications goals.

MANAGER OF INTEGRATED HEALTH SERVICES

Position Summary

The Manager of Integrated Health Services (MIHS) will ensure efficient administrative operation for the mental health and medical services provided at Callen-Lorde’s Article 31 Clinic. The MIHS will oversee daily operations and staffing and address patient concerns and complaints. The manager will also monitor visit volume, provide budget management and benchmark standards to develop corrective plans to improve service delivery. The MIHS will work in collaboration with the Director of Integrated Health and the Director of Psychiatry to ensure that clinical standards, funder, and regulatory compliance requirements are met. The MIHS will also conduct community outreach and develop partnerships to promote Callen-Lorde services to underserved populations. In addition, the MIHS will ensure the clinic is in compliance with all aspects of Article 31 and Article 28 service delivery documentation and contract deliverables for funding sources.

MEDICAL RECORDS MANAGER

Position Summary

Callen-Lorde is seeking a dedicated individual to join our team as the Medical Records Manager (MRM). This person will manage the day-to-day operations of the Medical Records department, insuring HIPAA-compliant handling of Health Information. The MRM will serve as Callen-Lorde’s Privacy Officer and oversee all privacy matters for the organization. The MRM will ensure the maintenance, integrity and security of the electronic and paper medical records system and coordinate the handling of PHI to ensure it meets New York State’s health information security and compliance standards. This person will be a working supervisor and lead a team responsible for responding to all requests for patient records, monitor telephone calls, lab entries, missing labs, scanning and filing. This person will directly address patient concerns and complaints. The MRM will contribute to the ongoing development, evaluation and improvement of Patient Services within the Medical Records department.

NEXTGEN EHR SPECIALIST

Position Summary

Callen-Lorde is seeking a flexible, dedicated individual to join our staff as a NextGen EHR Specialist (NES). The NES will develop, customize and test NextGen EHR templates and documents. The NES will troubleshoot issues related to the NextGen EHR and ensure that patient information is organized and maintained appropriately in the system. The NES will also ensure that templates and documents meet all internal and external requirements and regulations. This person will work in Callen-Lorde’s Health Information Technology (HIT) department and report to the Senior Director of Strategic Initiatives.

PATIENT CARE ASSOCIATE

Position Summary

PATIENT NAVIGATOR (BILINGUAL ENGLISH/SPANISH REQUIRED)

Position Summary

Callen-Lorde is seeking a dedicated, flexible individual to join our staff as a Patient Navigator. The Patient Navigator is responsible for providing field based case management and adherence support services to HIV-positive individuals who require help in accessing medical services. The Patient Navigator will work in a team setting headed by a Care Coordinator, in conjunction with medical and mental health providers. This is a field based position working with patients in their homes and accompanying them to medical and support services appointments. The goals of patient navigation activities are to ensure HIV-positive patients are linked to care in a timely and coordinated manner and to maintain patients in care via navigation, coordination of medical and social services and provision of support and coaching. Through teaching and supporting treatment (medication) adherence, patients should ultimately become self-sufficient and able to manage their medical and social needs autonomously.

REGISTERED NURSE, SCREENING (BILINGUAL ENGLISH/SPANISH REQUIRED)

Position Summary

The Registered Nurse is responsible for assisting primary care providers by screening patient phone calls and patient walk-ins for urgent medical matters, triaging patients to urgent care, and referring patients to the ER or other facilities. The RN will assist providers by providing education, adherence and case management support to patients. Other responsibilities will include following up on calls from the laboratory regarding alert values, providing education, information and support to patients regarding a variety of medical issues, including HIV and HIV adherence, and providing clinical support during patient care sessions when needed.

SOCIAL WORKER

Position Summary

The Social Worker will provide individual, family/couple and group psychotherapy sessions, supportive counseling, crisis intervention, care coordination, and referral services,. The position will work in collaboration with Callen-Lorde medical staff and consulting psychiatrists, as a member of our multi-disciplinary team. The overall goal of the position is to ensure that patients’ psychosocial needs are addressed and met.

TRANSGENDER MEDICAL CASE MANAGER

Position Summary

The Callen-Lorde Community Health Center seeks a dedicated, flexible individual to join our staff as a Medical Case Manager within our Health Outreach to Teens, or HOTT program. The HOTT Program provides low- and no-cost medical and mental health care to LGBTQ and homeless youth, ages 13-24. Youth services are provided both on-site and on our mobile medical unit. The Medical Case Manager is responsible for providing medical case management and supportive services to at risk youth, young adults and transgender patients accessing medical services within the HOTT program. This includes registration/data entry, financial screening, entitlements enrollment and referrals for youth accessing care at HOTT. In addition, this individual will provide family planning services and health education to a variety of HOTT patients (youth, young adults and transgender). The HOTT Medical Case Manger will work closely with primary medical care providers, adherence nurses, mental health and outreach workers to ensure that the bio-psychosocial continuum of care needs of HOTT clients are identified and appropriate services accessed, coordinated, and monitored. The Medical Case Manager plays a critical role in ensuring that patients achieve adherence to primary care recommendations and successfully transition to adult systems of care.
