CASE MANAGER – RETENTION AND ADHERENCE PROGRAM (SPANISH SPEAKING SKILLS REQUIRED)

Position Summary

The HIV Medical Case Manager will manage a case load of HIV-positive patients enrolled in the Retention and Adherence Program (RAP) and participate in all aspects of the agency’s Care Coordination program. Primary responsibilities include providing medical case management services which include; treatment education and adherence support, supportive counseling services, on-site case management, linkage and follow-up to on-site and external medical, mental and social services, the delivery of behavioral change interventions and DEBIs, oversight of Peer Educators and co-facilitation of youth and adult support groups. Additional the RAP Specialist will conduct intakes into the clinic for patients accessing HIV care. Responsibilities will also include internal data collection, reporting, and analyzing for contractual submission.

Schedule

Callen-Lorde’s main site in Chelsea is open Monday through Saturday. This is a full-time position with normal business hours (Monday – Friday, 9:00 – 5:00pm). Occasional evening hours will be required.

Qualifications

  • College degree (BA/BS) in health/social services preferred.

DATA PROGRAM COORDINATOR

Position Summary

Callen-Lorde is looking for a dedicated individual to join our Data team. The Data Program Coordinator (DPC) will work closely with the Health Outreach to Teens (HOTT) department and will be responsible for managing all grant related data reporting requirements. The DPC will coordinate all grant related data entry into relevant reporting systems and internal trackers and assure the quality and accuracy of this data. The DPC will assist with entering all grant related services into the AIDS Institute reporting system (AIRS), the New York City DOHMH reporting system (eSHARE), any other additional platforms for data reporting assigned by funders and assist in data collection and reporting as needed. The position will play a role in assuring grant service targets are being met, documentation is correct and up-to-date, and patients are being maintained in treatment, care and services as per grant protocols. The DPC will be responsible for the development and maintenance of all electronic health record templates, internal performance summaries, and raw data collection systems related to programs and contracts within the HOTT department. Additionally the DPC will develop strategic plans to improve all data entry and documentation, work with the management team to update data and documentation protocols, providing relevant documentation trainings to staff, and other administrative support functions.

Qualifications

  • Bachelor’s Degree in related field.

DIRECTOR OF HEALTH OUTREACH TO TEENS (HOTT) PROGRAM

Position Summary

Callen-Lorde Community Health Center is seeking a dedicated individual to join our management team as the Director of the Health Outreach to Teens (HOTT). The HOTT Program provides no– and low-cost medical and mental health care to LGBTQ youth, ages 13-24. The Director is responsible for the administrative operations, program development, quality management, marketing/outreach, and funding of the HOTT program. To this end, the Director oversees day-to-day clinical and administrative operations, both on-site and on the mobile medical unit. The Director leads the staff in the ongoing development, evaluation, and improvement of the program; coordinates quality assurance efforts in conjunction with clinical staff and other Callen-Lorde managers; coordinates outreach efforts to expand knowledge of HOTT within both the youth services community and communities of young people; and, provides leadership and prospecting for program funding, as well as management of funding contracts.

Qualifications

  • Master’s degree in social work or public health preferred; BS or BA in health administration or related field required
  • 5+ years of experience in social work or human services field. Health care experience preferred.
  • 3+ years of professional experience providing services to at risk adolescents, including assessment
  • Experience in non-profit management with a proven track record in successful implementation/development of programs for special needs populations
  • 3+ years of supervisory experience with strong skills in grant writing/reporting, program development, policy and procedures.
  • Experience managing multiple government contracts (AIDS Institute/Ryan White, and Family Planning/Title X)
  • Excellent oral and written communications skills
  • Strong organizational and time management skills

Position Summary

Qualifications

  • New York State Certification/Licensure (MD)

HOTT MEDICAL CASE MANAGER

Position Summary

Callen-Lorde is seeking a dedicated, flexible individual to join our staff as a Medical Case Manager (MCM) within our Health Outreach to Teens (HOTT) Program. The HOTT Program provides low- and no-cost medical and mental health care to LGBTQ and homeless youth, ages 13-24. The HOTT Program provides this care from a youth empowerment, low threshold, strengths-based, harm reduction and trauma-informed approach. Youth services are provided both on-site and on our mobile medical unit. The MCM is responsible for providing medical case management and supportive services to youth accessing medical services, with a focus on STI/HIV prevention & screening, Transgender Care, and HIV Care. This includes registration/data entry, financial screening, entitlements enrollment and referrals for youth accessing care at HOTT. The HOTT MCM will work closely with medical team to ensure that the biopsychosocial continuum of care needs of HOTT clients are identified, accessed, coordinated, and monitored. The MCM will facilitate a youth support group (ex: TGNC women’s, TGNC men’s, PrEP support, HIV+ support, CAB, and/or complimentary/holistic health group.) The MCM plays a critical role in ensuring that youth patients achieve adherence to primary care recommendations and successfully transition to adult systems of care.

Qualifications

MEDICAL SERVICES MANAGER

Position Summary

Callen-Lorde is seeking a dedicated and data-oriented individual to join our staff as a Medical Services Manager. The Medical Services Manager will be an integral part of the medicine department leadership team, and will work closely with the Directors of Medicine, Nursing, Clinical Operations and with individual medical providers. The Manager will develop and implement strategies, policies, and operational systems to effectively optimize resources and address patient needs. Additionally, the Manager will monitor visit volume and identify opportunities to maximize revenue through appointment utilization and billing practices.  The Manager will also develop corrective plans to improve outcomes and service delivery.

PHARMACY CONCIERGE

Position Summary

Callen-Lorde is seeking a dedicated, customer service oriented individual to join our staff as a Pharmacy Concierge. The Pharmacy Concierge will provide daily administrative support and serve as a liaison between our patients, the pharmacy, medical providers, mental health providers, and other staff at Callen-Lorde. The Pharmacy Concierge will help patients facilitate their requests for new prescriptions, prescription refills and make sure all necessary information including insurance verification is available to process orders. The Pharmacy Concierge will also educate patients of all pharmacy programs and services. PREVENTION OUTREACH PROVIDER

Position Summary

The Prevention Outreach Provider (POP) will participate in all aspects of the agency’s prevention program.  Responsibilities include providing screening and assessment/risk reduction counseling to patients in the Sexual Health Clinic as well as rapid HIV testing in accordance with established Callen-Lorde testing protocol.  In addition, the POP will help patients navigate access to PrEP, and conduct targeted outreach, testing, and risk reduction counseling to trans and gender non-conforming patients. The POP will also participate in Callen-Lorde’s community outreach, education and marketing activities, through community events in the New York metropolitan area, and through collaborations to other CBOs.

SQL REPORT WRITER

Position Summary

Callen-Lorde is seeking an SQL Report Writer to perform standard and ad-hoc reporting and data analysis based on the provision of healthcare services at Callen-Lorde for internal and external clients. Working with managers and clinical staff this individual will gather report requirements and utilizing specialized reporting tools, develop reports based on data in the Electronic Health Record (EHR) and Practice Management (PM) database and other sources. This individual will also propose new reports as needed, and will provide guidance to staff in running and interpreting such reports.

Callen-Lorde’s Administrative Office is open Monday through Saturday. This is a full-time position with normal business hours (9:00AM to 5:00PM, Monday – Friday).

Qualifications

  • Bachelor’s degree – Information Systems or related field preferred
  • Strong working knowledge of SQL required
  • 2-5 years of experience with report development tools (SQL Server Reporting Services (SSRS) 2008 R2 or newer required
  • Experience working in the healthcare industry required, especially EHR/EMR and practice management data analysis, clinical databases, diagnostic coding, and relevant data privacy regulations
  • Design and development experience using Microsoft’s Business intelligence stack preferred (SQL Server Integration Services (SSIS), SQL Server Analysis Services (SSAS), SharePoint reporting/dashboards, PowerView, and Excel PowerPivot)
  • Commitment to data quality and consistent reporting
  • Ability to design and lead reporting projects involving end users
  • Experience with Crystal Reports XI or newer preferred
  • Excellent business writing and technical documentation skills
  • Strong oral communication skills
  • Sensitivity to LGBT, HIV, and diverse communities required
  • Ability to handle multiple, simultaneous tasks, and excellent attention to detail
