ASSOCIATE DIRECTOR OF MEDICINE – CALLEN-LORDE BRONX

Position Summary

Callen-Lorde is seeking an experienced and dedicated medical provider (MD, DO, NP) to join our team as an Associate Director of Medicine (ADM) at our Bronx Clinic. The ADM will collaborate with the Director of Medicine in providing clinical and administrative leadership and vision to Callen-Lorde’s Bronx Clinic, particularly with primary care services. The ADM is responsible for the professional, organizational, and administrative aspects of the quality of care provided to patients, and operates within the framework of Callen-Lorde’s mission and applicable city, state, and federal mandates at Callen-Lorde’s Bronx location. The ADM will also have their own patient panel.

Schedule

Callen-Lorde’s Bronx site is open Monday through Saturday. This is a full-time position that will require three 8:30-4:30pm shifts and two 12:30-8:30pm shifts.

Qualifications

  • New York State (MD, DO, NP) License and Registration

BEHAVIORAL HEALTH COORDINATOR*

Position Summary

The Coordinator of Behavioral Health (Coordinator) works closely with the management and clinical teams to ensure appropriate access to health care for patients at Callen-Lorde’s 17th Street Clinic. The Coordinator is responsible for the oversight of the front desk staff, administrative services, patient scheduling and pre-screening process for the mental health and primary care clinic. The Coordinator will assist with patient tracking, template management, data reporting and practice management. The Coordinator will administer staff trainings and ensure excellent customer service is being provided to all patients. In addition, this person will approve staff timesheets and time off requests. The coordinator will play a key role in providing administrative support to both the management and clinical teams.

Schedule

Callen-Lorde’s 17th Street Clinic is open Monday through Friday. This is a full-time position with normal business hours. Occasional evening and weekend hours may be required.

Qualifications

  • BS/BA degree in Social Services related discipline

BILLING ASSOCIATE

Position Summary

Callen-Lorde is seeking a friendly and organized individual with billing and data entry experience in medical settings to join our staff as a Billing Associate (BA). The BA is responsible for data entry and maintaining an efficient flow and accurate accounting of patients through the various medical, mental health and oral health services within Callen-Lorde. The BA must be courteous, patient, and professional at all times. Daily responsibilities will include patient accounts management, registration/data entry, financial screening, entitlements enrollment and referral, insurance verification, claims submission, re-billing/corrections, transaction adjustments, payment entry and reconciliation.

Schedule

This is a full-time position with normal business hours (Monday-Friday, 8:00-5:00pm).  Occasional evenings will be required.

CARE COORDINATOR*

Position Summary

CASE MANAGER – HOTT*

Position Summary

Callen-Lorde is seeking a dedicated, flexible individual to join our staff as a Case Manager (CM) within our Health Outreach to Teens (HOTT) Program. HOTT provides low- and no-cost medical and mental health care to LGBTQ and homeless youth, ages 13-24. This position will involve both onsite and field based medical case management and supportive services to young people, with a focus on STI/HIV prevention & screening, Transgender Care, and HIV Care. The CM will work closely with the medical team to ensure that the biopsychosocial continuum of care needs of HOTT clients are identified, accessed, coordinated, and monitored. The CM will accompany patients to appointments on an as needed basis. The CM plays a critical role in ensuring that HOTT patients achieve adherence to primary care recommendations and successfully transition to adult systems of care.

CLINICAL INFORMATICS SPECIALIST (NP, PA)

Position Summary

We are seeking a Licensed Clinician to work in a support role in the Health Information Technology Department improving the functionality of our EMR. The Clinical Informatics Specialist (CIS) will work to assess current workflows, processes and implementing improvements and/or enhancements to NextGen’s electronic health record, practice management system and dental record. Working with a NextGen multi-disciplinary team they will help evaluate potential HIT solutions based on clinical requirements. They will engage relevant clinical staff to document new workflows and ensure close coordination with the data reporting teams so that reports capture changes. They will also assist with training staff and performing post-implementation monitoring of the adoption and use of new or enhanced workflows. The clinician in this support role will (ideally) potentially also maintain a very limited clinical practice, (1 day per week) in order to have hands on experience with the EMR.

Qualifications

  • A Master of Science in Nursing or Physician Assistant Program.

COMMUNICATIONS ASSOCIATE

Position Summary

Callen-Lorde is seeking an organized, tech-savvy, and motivated Communications Associate who will be responsible for developing and coordinating communications for patients and communities at large, through print and digital health awareness campaigns, and both internal and external events. In this role, the Communications Associate is also responsible for creating and updating health literate, multilingual content including printed materials such as flyers and brochures, and digital content such as blogs, social media posts, and digital signage. The Communications Associate will also lead Callen-Lorde’s Pride efforts, managing all events and activities, serving as liaison between Pride organizers and staff/community members. FACILITIES MANAGER

Position Summary

Callen–Lorde is seeking an experienced professional to oversee the provision of services to our growing organization that has several medical clinics and administrative sites. In this role, the Facilities Manager will oversee facilities operations services and maintenance, workplace environmental health & safety, regulatory compliance, and security. The Facilities Manager will ensure that all staff have a safe, clean, healthy and comfortable work environment. The Facilities Manager will also have over site of Callen-Lorde’s mobile medical van. Additionally, the Facilities Manager will lead facilities relocation and expansion projects of considerable scope and complexity. In this role the Facilities Manager will also manage relationships with external contractors and ensure all vendors institute effective systems for logging and tracking of needed inspections, maintenance schedules, and repair needs. The Facilities Manager will also take the lead in communicating timelines for resolution of problems with staff. GENERAL DENTIST*

Position Summary

Callen-Lorde is seeking an experienced, dedicated and reliable General Dentist to join our team in providing comprehensive and appropriate oral health services to a diverse patient population, including those living with HIV/AIDS. In this role, the Dentist will perform routine preventative care and maintenance dentistry in a fast-paced and high-volume environment. The Dentist will perform complex treatments and rehabilitation procedures for advanced oral diseases and injuries of teeth and gums and will also provide emergency dental services, including diagnosis and treatment plans, general restorative procedures, crown and bridge fabrication, removable prosthetics, uncomplicated endodontics, minor oral surgery and initiate/interpret radiographs. In addition, the Dentist will administer and prescribe anesthetics and medications, refer patients for specialty care to other clinic programs or outside agencies, conduct follow-up patient care, provide patient education and provide supervision to Dental Hygienist(s) and Dental Resident(s).

MANAGER, CARE COORDINATION

Position Summary

Callen-Lorde is seeking a dedicated, individual to join our staff as the Manager of the Care Coordination Department. The Manager oversees several Care Coordination programs facilitating case management services for the full spectrum of Callen-Lorde’s patient population. The Manager will work in collaboration with medical providers, nursing, and behavioral health staff to ensure that the full continuum of care for our patients are identified, implemented and monitored. The Manager will support the Director of Care Coordination in providing administrative and programmatic support to all Care Coordination programs, including budgeting, reporting, chart reviews, and will serve as a liaison with grantors.

Schedule

Callen-Lorde’s main site in Chelsea is open Monday through Saturday. This is a full-time position that will require morning, daytime and evening shifts. Occasional weekends may be required.

Qualifications

  • Licensed Master of Social Work Degree (LMSW) or Licensed Mental Health Counselor (LMHC) preferred. Related Master’s degree and/or 5 + years of significant and relevant work history will be considered.
  • Prior experience supervising case management staff and performance-based grant contract monitoring required (Ryan White preferred).
  • A minimum of 2 years experience in providing direct service case management/ treatment adherence services to target populations including, patients of transgender experience, lesbians, communities of color, MSM, and/or those living with HIV.
  • Strong computer competency, including knowledge of Electronic Medical Records, Computerized Appointment/Billing Programs and Microsoft Office Programs.
  • Prior experience in an outpatient health care setting desired.
  • Extensive knowledge of public assistance, social services and related entitlement programs.

MOBILE MEDICAL UNIT DRIVER – PER DIEM

Position Summary

Callen-Lorde is seeking a per diem driver for our Mobile Medical Unit. The Mobile Medical Unit provides comprehensive medical care to young people in our Health Outreach to Teens program. The Driver must be available to work on Thursday and Friday evenings between 10pm and 3am. There may be other evening and weekend opportunities available as well. The position is currently a per diem position, but it may lead to full-time employment in the future.  Valid Commercial Driver’s License is required. On occasion the MMU Driver will be responsible for light maintenance of the mobile medical unit and will verify that all vehicle systems are operational.  The Driver will provide security for all medical and office supplies as well as MMU logs and address on-site security issues, when appropriate.

Schedule

Callen-Lorde’s main site in Chelsea is open Monday through Saturday. This is a per diem position that will include Thursday and Friday evenings between 10pm and 3am.  Salary will be based on experience.

POPULATION HEALTH DATA ANALYTICS MANAGER

Position Summary

The Population Health Analytics Manager (PHAM) will direct clinic-wide initiatives that aim to help Callen-Lorde understand its trends, indicators, and provider performance through data. The PHAM will supervise a small staff of Population Health Coordinators and will also collaborate closely with Health Information Technology staff to create innovative systems for data capturing and reporting. The PHAM will respond to data requests, build customized reports, and help develop a culture of data literacy throughout the clinic. The PHAM will also work with the management team to update protocols, provide documentation trainings to staff, and perform other administrative support functions.

SOCIAL WORKER, LCSW – IN-HOME FAMILY THERAPIST*

Position Summary

Social Workers at Callen-Lorde provide individual, family, couple and group psychotherapy sessions, supportive counseling, crisis intervention, care coordination, and referral services. The In-Home Family Therapist will work in a grant-funded position to deliver in-home family strengthening services to patients. At the start, the provider will conduct in-home work two days per week, and work in the behavioral health clinic 3 days per week. Once funding is secured, the provider will work 5 days per week conducting in-home family and individual therapy.
