CASE MANAGER, RETENTION & ADHERENCE PROGRAM (BILINGUAL SPANISH REQUIRED)

Position Summary

The HIV Medical Case Manager will manage a case load of HIV-positive patients enrolled in the Retention and Adherence Program (RAP) and participate in all aspects of the agency’s Care Coordination program. Primary responsibilities include providing medical case management services which include; treatment education and adherence support, supportive counseling services, on-site case management, linkage and follow-up to on-site and external medical, mental and social services, the delivery of behavioral change interventions and DEBIs, oversight of Peer Educators and co-facilitation of youth and adult support groups. Additional the RAP Specialist will conduct intakes into the clinic for patients accessing HIV care. Responsibilities will also include internal data collection, reporting, and analyzing for contractual submission.

Schedule

Callen-Lorde is open Monday – Saturday.  Evening and Saturday hours will be required.

Qualifications

  • College degree (BA/BS) in health/social services preferred.
  • Experience in health care, HIV service delivery or social services preferred.
  • Experience in case management and knowledge of HIV medications and co-morbidities preferred.
  • Bilingual (Spanish/English) required.
  • Certification in Phlebotomy preferred.
  • Commitment to the mission of Callen-Lorde Community Health Center.
  • Ability to work effectively with diverse populations, including LGBT communities.
  • Demonstrated knowledge of HIV/AIDS, Sexually Transmitted Infections (STIs) and transmission risks.

DATA PROGRAM ASSISTANT

Position Summary

Callen-Lorde is seeking a Data Program Assistant (DPA) responsible for entering all grant related data into relevant reporting systems and internal trackers and assuring the quality and accuracy of this data. The DPA will also enter all grant related services into the AIDS Institute reporting system (AIRS), the Center for Disease Control reporting system Evaluation Web and the New York City DOHMH reporting system (eSHARE). The DPA will assist in data collection and reporting as needed, and participate in data mining tasks. The position will play a role in assuring grant service targets are being met, documentation is correct and up to date, and patients are being maintained in treatment, care and services as per grant protocols. Further responsibilities will include other administrative support functions determined by the Data Program Manager and other relevant management.

Schedule

Callen-Lorde’s main site in Chelsea is open Monday through Saturday. This is a full-time position with normal business hours (Monday – Friday, 9:00 – 5:00pm). Occasional evening hours will be required.

Qualifications

  • Bachelor’s Degree preferred

DENTAL ASSISTANT

Position Summary

Callen-Lorde is seeking an exceptionally friendly individual with excellent dental clinical skills to join our staff as a Dental Assistant. Dental Assistants provide clinical support to dentists and hygienists. Responsibilities include chair side assistance; radiology and laboratory assistance and preparation of patients. Duties also include monitoring and stocking supplies, infection control and room maintenance. Additionally, Dental Assistants are responsible for ensuring appropriate and efficient patient flow for the Dental Providers.

Callen-Lorde’s main site in Chelsea is open Monday through Saturday. This is a full-time position with morning, afternoon and evening hours.

Qualifications

  • Dental Assistant Certification Required
  • High school diploma or equivalent required

DENTAL HYGIENIST, PART-TIME

Position Summary

Callen-Lorde is seeking a part-time Dental Hygienist to provide quality dental care services to patients under the general supervision of our dentists and using established dental hygiene procedures. The Dental Hygienist is responsible for providing preventative dental services to patients in a sensitive and non-judgmental manner. The Dental Hygienist is expected to assess the dental condition and needs of patients using approved screening procedures, and provide recommended treatment while maintaining a high degree of professional competence and the highest possible ethical standards, ensuring patient confidentiality and function as part of a care-giving team. The Dental Hygienist operates under Callen-Lorde’s protocols and under the supervision of the Dental Director.

Schedule

The position is part-time and will only consist of one scheduled day per week (Tuesdays, 8:30am –4:30pm). Additional hours may be considered based on the needs of the dental clinic.

Qualifications

  • Current NYS required Licensure/Certification/Registration.
  • CPR certification.
  • Minimum of 2 years of experience as a Dental Hygienist.
  • Ability to work in a high-volume clinic.
  • Ability to work within team-based structure.

GRANTS ASSOCIATE

Position Summary

Callen-Lorde is seeking a Grants Associate responsible for the research, development, writing and submission of proposals to increase grant revenues for Callen-Lorde Community Health Center from local, state and Federal government sources. The Associate will further support grant tracking and management in close collaboration with program managers and staff.  The Associate will manage the proposal development process from beginning to end as well as track all grants submitted throughout the funding year.  The Associate will work with their supervisor, program teams and grants procurement team to ensure all potential grant opportunities are researched and submitted in a timely manner.  The Associate will report to the Director of Project Management, and work closely with the Director of Fund Development and Special Projects as well as managers and staff of grant-funded programs, who oversee more than $6 million in contracts. The Associate will work alongside the organizations Development department, which manages foundation relations.

Qualifications

  • Bachelor’s degree and 2-3 years of experience as a grant writer or grant manager, with a proven record of success in acquiring local, state and Federal grants. Prior experience writing grants for health related funding from both public and private sources strongly preferred.
  • Excellent written and verbal communication skills and a proven ability to write under pressure.
  • Must be detail oriented and have the ability to manage several writing projects simultaneously.
  • Excellent interpersonal skills, including ability to work with staff at all levels, interact with funders, and represent the organization tactfully and diplomatically.
  • Excellent research skills, preferably with experience in researching government, and corporate funders using online databases and the Internet.
  • Knowledge of Microsoft Office is required.

IT PROJECT MANAGER

Position Summary

Callen-Lorde is seeking a dedicated, flexible individual to join our team as the IT Project Manager. The IT Project Manager (ITPM) will be responsible for overseeing the rollout of multiple projects, across the organization, utilizing electronic medical records (EMR), technology infrastructure and analytics, Smartsheet and/or Microsoft Project software.  Primary responsibilities include planning and evaluating projects, ensuring projects are on schedule, assigning duties to staff members, implementation of projects and collaborating with managers.  This person will also be responsible for report writing and reviewing budgeted project expenditures.  The goals of the ITPM are to foster continuous improvement within the growing organization, increase organizational and staff skills in group development of interdisciplinary work products and to institutionalize sound project management methodology throughout the organization.  The ITPM will report to the Senior Director of Innovation, Informatics and Quality.

Schedule

Callen-Lorde’s Administrative Office is open Monday through Friday. This is a full-time position with normal business hours (9:00AM – 5:00PM).

Qualifications

  • Bachelors in Management or healthcare related field and a minimum of 5-7 years of project management experience.
  • Healthcare IT experience required.
  • Project Management certification preferred but not required.
  • Ability to work collaboratively with peers across multiple departments.
  • Ability to manage ambiguous situations and adapt to constantly changing priorities in managing a wide range of projects.
  • Strong computer skills, including familiarity with database systems and MS Project, MS Suite, as well as knowledge and experience working with electronic medical records systems preferred

PATIENT NAVIGATOR (BILINGUAL SPANISH)

Position Summary

Callen-Lorde is seeking a dedicated, flexible individual to join our staff as a Patient Navigator. The Patient Navigator is responsible for providing field based case management and adherence support services to HIV-positive individuals who require help in accessing medical services.  The Patient Navigator will work in a team setting headed by a Care Coordinator, in conjunction with medical and mental health providers. This is a field based position working with patients in their homes and accompanying them to medical and support services appointments. The goals of patient navigation activities are to ensure HIV-positive patients are linked to care in a timely and coordinated manner and to maintain patients in care via navigation, coordination of medical and social services and provision of support and coaching. Through teaching and supporting treatment (medication) adherence, patients should ultimately become self-sufficient and able to manage their medical and social needs autonomously.

PAYROLL ADMINISTRATOR

Position Summary

Callen-Lorde is seeking a Payroll Administrator responsible for accurately processing employee data (new hires, changes, additional earnings) into the payroll system and processing a bi-weekly payroll for amount a staff of 300. Additional responsibilities include the review and importing employee time & attendance data from the time system to payroll and reconciling variances via excel, creating ad-hoc reports, and processing benefit deductions for 403b plan and other benefit programs. The Payroll Administrator will support the work of the Senior Accountant in vouchering for grant contracts, including time and effort attestations. In addition, this employee will backup accounts payable duties including, coding of invoices to cost centers and funding sources, invoice accuracy, and check preparation.

Schedule

This is a full-time position, Monday through Friday, with occasional evening hours.

Qualifications

  • Bachelor or Associates Degree required in Finance or Accounting preferred.
  • 2-3 years of payroll experience, preferably in a union setting.
  • Formal Payroll training/certification is a plus.
  • Grants administration is preferred.
  • Experience in Solomon and Paychex software strongly preferred.
  • Advanced knowledge of MS Office (Excel, Word, and Access) required.

PROGRAM ASSISTANT – FACILITATED ENROLLMENT (BILINGUAL SPANISH)

Position Summary

Callen-Lorde is seeking a dedicated individual to join our staff as a Program Assistant (PA) within the Facilitated Enrollment department.
