HEPATITIS C PROGRAM COORDINATOR, NURSE PRACTITIONER OR PHYSICIAN’S ASSISTANT

Position Summary

Callen-Lorde is seeking a Hepatitis C Program Coordinator who will be part of Callen-Lorde’s multidisciplinary Hepatitis C Program and functions under the supervision of the Senior Director of Research and Education. The Medical Provider in this position provides direct treatment and assists with Hepatitis C linkage to care, and related logistics/care coordination, and data collection and reporting. The coordinator will assess patients for treatment readiness and adherence. The coordinator will also be responsible for ongoing tracking of all Hepatitis C positive patients. The coordinator will assist with Hepatitis C related education and training activities for patients and staff.

Schedule

Callen-Lorde’s main clinical site in Chelsea is open Monday through Saturday.  This is a full-time position that will require both morning and evening shifts.

Qualifications

  • New York State License and Registration (NP or PA)
  • Board Certification and AAHIVM certification preferred
  • Current DEA Registration required
  • Current CPR and Infection Control certification required
  • Minimum 1-2 years of acute or ambulatory care experience is required, additional experience highly preferred.
  • At least one year of experience with Hep-C and/or HIV treatment in a public/community health care setting is required.
  • Experience in sexual health and in screening, treating & patient education in sexually transmitted infection within a harm reduction model of care
  • Case Management and patient adherence experience preferred
  • Ability to work with diverse patient populations and within a team-based structure

MEDICAL PROVIDER – HIV SPECIALIST

Position Summary

Callen-Lorde is seeking an experienced, dedicated medical clinician (MD, DO, NP) to join our team in providing both HIV and primary care services to a diverse panel of patients.  Working with a close-knit group of colleagues, the medical provider will maintain a high degree of professional competence and functions as a reliable member of a patient care team.

PAYROLL ADMINISTRATOR

Position Summary

Callen-Lorde is seeking a Payroll Administrator who will be responsible for processing accurately employee data (new hires, changes, additional earnings) into the payroll system and processing a bi-weekly payroll for amount a staff of 300.  Additional responsibilities include the review and importing employee time & attendance data from the time system to payroll and reconciling variances utilizing tools in excel. You will create ad-hoc reports and process benefit deductions for 403b plan and other benefit programs.  The Payroll Administrator will support the work of the Senior Accountant in vouchering for grant contracts, including time and effort attestations.  In addition, this employee will backup accounts payable duties including, coding of invoices to cost centers and funding sources, invoice accuracy, and check preparation.

Schedule

The position is scheduled to work Monday through Friday (9:00am – 5:00pm), with occasional evening hours.

Qualifications

  • Bachelor or Associates Degree required in Finance or Accounting preferred
  • 2-3 years of payroll experience, preferably in a union setting
  • Formal Payroll training/certification is a plus
  • Grants administration is preferred
  • Experience in Solomon and Paychex software strongly preferred
  • Advanced knowledge of MS Office (Excel, Word, and Access) required

  • Strong customer service orientation and ability to manage vendor relations.

PHARMACY TECHNICIAN

Position Summary

Callen-Lorde is seeking a dedicated and flexible individual to join our staff as a Pharmacy Technician (PT). Responsibilities will include processing requests for prescription renewals, prior authorizations, and serving as a liaison between patients, medical providers and pharmacies. The PT will ensure patient confidentiality (HIPAA regulations) are adhered to and will function as part of a care-giving team. The PT will also ensure that mail ordered medications are properly logged and managed in Callen-Lorde’s electronic medical record. PTs are expected to work in a fast paced, high volume, team based environment while maintaining a high degree of professional competency and ethical standards. The PT will work under the supervision of the Nursing Operations Manager.

Qualifications

PREVENTION AND OUTREACH PROVIDER

Position Summary

Callen-Lorde is seeking a dedicated Prevention and Outreach Provider (POP) to participate in all aspects of the agency’s Prevention and Outreach program.  Responsibilities will include providing HIV testing, screening and assessment/risk reduction counseling to patients.  Responsibilities will also include internal data collection, reporting, and analyzing for contractual submission. In addition, the POP will participate in Callen-Lorde’s community outreach, education and marketing activities, through community events in the New York metropolitan area. The POP will also assist with the design, implementation and maintenance of multiple prevention interventions unique to Callen-Lorde.

REGISTERED NURSE, SCREENING

Position Summary

Callen-Lorde is seeking a Screening RN responsible for assisting primary care providers by screening patient phone calls and patient walk-ins for urgent medical matters, triaging patients to urgent care, and referring patients to the ER or other facilities.  The RN will assist providers by providing education, adherence and case management support to patients.  Other  responsibilities will include following up on calls from the laboratory regarding alert values, providing education, information and support to patients regarding a variety of medical issues, including HIV and HIV adherence, and providing clinical support during patient care sessions when needed.

SOCIAL WORKER

Position Summary

Callen-Lorde is seeking a Social Worker to provide individual, family/couple and group psychotherapy sessions, supportive counseling, crisis intervention, care coordination, and referral services. The position will work in collaboration with Callen-Lorde medical staff and consulting psychiatrists, as a member of our multi-disciplinary team. The overall goal of the position is to ensure that patients’ psychosocial needs are addressed and met. The social worker may be required to provide therapy on a walk-in basis in our Article 31 clinic, rather than having a set patient schedule and/or providing short/long term therapy depending on the needs of the clinic.

Schedule

Callen-Lorde’s Behavioral Health Clinic is open Monday through Friday. This is a full-time position which will include some evening hours.

Qualifications

  • Master’s degree in Social Work and NYS licensure – LMSW or LCSW required.
  • At least 1 or more years post-Master’s clinical experience working with both adult individuals and groups preferred.
  • Experience working in New York State Article 31 outpatient practice setting preferred.
  • Knowledge of LGBT and HIV-related health/behavioral health issues and concerns.
  • The ability to maintain on-going documentation in an electronic charting system.
  • Strong computer competency.
  • Bilingual (Spanish/English) preferred.
  • Experience serving Callen-Lorde’s target populations preferred +/or a strong commitment to the mission of Callen-Lorde.
