HUMAN RESOURCES RECRUITER

Position Summary

Callen-Lorde is seeking a Human Resources professional to join our team and support our growing organization. The HR Recruiter will work closely with Callen-Lorde’s Management team and perform the full cycle of recruitment for multiple sites. The Recruiter will work to acquire talented, mission driven candidates for a broad range of healthcare positions including administrative/service orientated roles, clinical professionals and Management level positions. The Recruiter will redesign/automate Callen-Lorde’s hiring process and implement an online application and screening process. In addition, the Recruiter will create/edit job postings, screen applicants, schedule/conduct interviews, extend offers, perform reference checks and facilitate the onboarding process for new hires. LICENSED PRACTICAL NURSE

Position Summary

Callen-Lorde is seeking a full time bilingual Licensed Practical Nurse as we open a new site in the Bronx, but will also work in our main facility located in Manhattan as directed by the Management Team. The LPN will be responsible for assisting primary care providers — physicians and mid-level providers — in providing patient care by providing general nursing, assistance with triage, patient service and clinical support. He/she/they are expected to maintain a high degree of professional competence and the highest possible ethical standards, ensuring confidentiality and functioning as part of a care-giving team. The LPN operates under strict protocols and under the supervision of the Director of Nursing.

MEDICAID BILLING ASSOCIATE

Position Summary

We are seeking an exceptionally friendly, organized individual with computer proficiency and billing/data entry experience in medical settings to join our staff as a Patient Accounts Associate (PAA). The PAA is responsible for heavy data entry and maintaining an efficient flow and accurate accounting of patients through the various medical, mental and oral health services and their related financial systems within the Center. As the first contact with our patients, the billing associate must be courteous, patient, and professional at all times. The realm of daily work spans the continuum of patient accounts management–appointment scheduling, registration/data entry, financial screening, entitlements enrollment and referral, insurance verification, patient check-in/out, cashiering, claims submission/re-billing/corrections, transaction adjustments and payment entry and reconciliation.

NEXTGEN SUPPORT ANALYST

Position Summary

Callen-Lorde is seeking a flexible, dedicated individual to join our staff as a NextGen Support Analyst (SA). The SA will serve as a technical resource for the health center’s NextGen Practice Management (PM), Electronic Health Record (EHR), Electronic Dental Record (EDR) and Document Management (formerly ICS) systems. The SA will work within the Health Information Technology (HIT) team to provide end user support. In this role the SA will track all HIT related support requests assuring that an appropriate member of the team is following up on any issues in a timely manner, this will include monitoring all support tickets opened with NextGen to assure their timely response and escalation of issues as necessary. In this role the SA will identify and troubleshoot issues within NextGen systems, working to resolve them by identifying existing NextGen resources and independent investigation, the SA will perform all follow-up activities to assure satisfactory resolution of issues and communication with the end users. The SA will participate in the implementation of all system software enhancements and upgrades. This position may also include EHR template and document development as well report development in EHR and PM systems.

NURSING MANAGER

Position Summary

Callen-Lorde is seeking a Nursing Manager who will work directly with the Director of Nursing to oversee and supervise nursing services at Callen-Lorde. In this role, the Nursing Manager will be responsible for ensuring smooth patient flow in the medical clinic. Responsibilities will include supervising the Nursing Coordinators, managing schedules and monitoring the performance of 30+ LPNs, MAs, Phlebotomists and Pharmacy Technicians. Additionally, the Nursing Manager will support the Director of Nursing in the development of department protocols, policies, procedures and standards of care. Position Summary

Callen-Lorde is seeking exceptionally friendly and detail oriented individuals to join our staff as Patient Care Associates (PCA). PCAs are a part of our dynamic medical team and work in collaboration with our nursing and provider staff to ensure optimal patient care. PATIENT NAVIGATOR (SPANISH SPEAKING REQUIRED)

Position Summary

Callen-Lorde Community Health Center is seeking a dedicated, flexible individual to join our staff as a Patient Navigator. The Patient Navigator is responsible for providing field based case management and adherence support services to HIV-positive individuals accessing medical services. The Patient Navigator will work in a team setting headed by a Care Coordinator, in conjunction with medical and mental health providers, to ensure that patients maintain stable health status. This is a field based position working with patients in their homes and accompanying them to appointments, as such the schedule is determined by patient needs. The goals of patient navigation activities are to ensure HIV-positive patients are linked to care in a timely and coordinated manner and maintain patients in care via navigation, coordination of medical and social services and provision of support and coaching. By teaching and supporting treatment (medications) adherence patients will become self-sufficient and able to manage their medical and social needs autonomously.

PATIENT SUPPORT REPRESENTATIVE

Position Summary

Callen-Lorde is seeking a Patient Support Representative (PSR) to serve as an initial point of contact for patient feedback and will be responsible for ensuring excellence in service delivery recovery and in resolution of patent concerns and issues. To this end, s/he carries out or facilitates the resolution of patient issues, including communication with patients in person, by phone, and/or in writing. The PSR assists the Patient Support Manager in the investigation, resolution and documentation of patient feedback; facilitates the development of action plans to address patient feedback and suggestions; facilitates patient satisfaction surveys and other patient feedback mechanisms; and, participates in other continuous quality improvement efforts. PREVENTION OUTREACH PROVIDER

Position Summary

Callen-Lorde is seeking a Prevention and Outreach Provider (POP) who will participate in all aspects of the agency’s Prevention and Outreach program. Responsibilities include providing HIV/HCV/STI testing, linkage to nPEP and PrEP, and screening and assessment/risk reduction counseling to patients in both standalone visits and the Sexual Health Clinic, in accordance with the established Callen-Lorde testing protocol, and providing counseling related interventions through related funded grants and contracts including but not limited to CLEAR, and Personalized Cognitive Counseling (PCC). Responsibilities will also include internal data collection, reporting, and analyzing for contractual submission. In addition, the POP will participate in Callen-Lorde’s active recruitment and referrals for related prevention programs, research and services, community outreach, education and marketing activities, through community events in the New York metropolitan area and collaborations with other CBOs. Responsibilities will also include involvement in the design, implementation and maintenance of multiple prevention interventions unique to Callen-Lorde. This position will provide services to all Callen-Lorde patients with a focus on working with young transgender individuals of color and their partners (with special interest in fostering a strong presence within the House and Ball community), working with relevant research studies and the application of biomedical interventions within an integrated health care setting.

PSYCHIATRIST/PSYCHIATRIC NURSE PRACTITIONER – CALLEN-LORDE AT BOOM!HEALTH, BRONX (PER DIEM)

Position Summary

Callen-Lorde is seeking an experienced, dedicated Psychiatrist or Psychiatric Nurse Practitioner at Callen Lorde’s new location in the Bronx. This position requires demonstrated competence in meeting the mental health needs of the LGBT community and persons living with HIV in compliance with NYS DOH regulations. This position will provide psychiatric assessment and psychopharmacological treatment of patients with acute and chronic psychiatric illnesses. You will become part of an integrated team providing mental health and medical services to a wide array of patients.

Schedule

This position requires 1 full day per week from 10 am – 6 pm. Salary will depend on experience. Choice of days is open (Monday, Tuesday, Wednesday, Thursday or Friday).

Qualifications

  • Bilingual (Spanish/English) strongly preferred.
  • New York State Licensure and current New York State Registration as a Psychiatrist or Psychiatric Nurse Practitioner.
  • Board Certification required for Psychiatrist.
  • Minimum of 2 years prior experience required.

SOCIAL WORKER, CALLEN-LORDE AT BOOM!HEALTH BRONX (LCSW and SPANISH SPEAKING REQUIRED)

Callen-Lorde is entering our next exciting phase of significant expansion as we open a new site in the Bronx. We are seeking an LCSW to join our team serving clients of BOOM!Health as well as the local community, with a focus on homeless populations, particularly those of transgender/gender non-conforming experience. While integrated both administratively and clinically within Callen-Lorde’s other sites, this site operates as a stand-alone FQHC.

Position Summary

Callen-Lorde is seeking a Social Worker to primarily provide short term individual therapy, assessment, supportive counseling, crisis intervention, and care coordination in a combination of an open access and scheduled appointment model. The position will work in collaboration with Callen-Lorde medical staff and consulting psychiatrist, as a member of our multi-disciplinary team. The overall goal of the position is to ensure that patients’ psychosocial needs are addressed.

Qualifications

  • Master’s degree in Social Work and NYS licensure-LCSW required.
