CLINICAL DIRECTOR, HEALTH OUTREACH TO TEENS (HOTT)
Position Summary
The Clinical Director of HOTT is responsible for overseeing the delivery of comprehensive primary care to our adolescent patients, including HIV+, homeless, and questioning youth, in a sensitive and non-judgmental manner. The Director is expected to maintain a high degree of professional competence and the highest possible ethical standards and to function as clinical leader of a care-giving team. Administrative responsibilities include the training, education, and clinical supervision of staff providing adolescent care, the development of clinical protocols in conjunction with the Clinical Director of Medical Services, management of youth related grant funded program requirements, and participation in quality management. The Director will function as part of a team to develop strategic growth initiatives for HOTT.
Work Schedule/Salary/Benefits
Callen-Lorde is open Monday through Saturday. This full-time position requires weekly 12 noon – 8:00 pm shifts, as well as Saturdays on rotation. Qualifications
  • Current New York State license and registration as a MD, DO, or FNP.
  • Qualification as HIV specialist by NY DOH AIDS Institute standards.
  • DEA Controlled Substances registration certificate and BCLS/ACLS certification required.
  • At least 3 years of clinical HIV and adolescent experience required, preferably working with lesbian, gay, bisexual, and/or transgender populations.
  • Experience in clinical administration strongly preferred.

Position Summary

DIRECTOR OF DEVELOPMENT AND COMMUNICATIONS
Position Summary
The Director of Development and Communications (Director) develops, oversees, and executes Callen-Lorde's fundraising and external communications efforts. The Director is responsible for the development and successful implementation of Callen-Lorde's fundraising programs, working closely with the Executive Director and the Board of Directors. Fundraising initiatives include individual giving, major donors, special events, and foundation/corporate giving. The Director will be expected to play an especially hands-on role in the growth of the major donor program. The Director is also responsible for development and delivery of an external communications strategy that increases awareness and understanding of Callen-Lorde's mission and services to the community at large, the media, and governmental bodies. The Director provides guidance and supervision to staff members and works with them as a team to achieve fundraising and communications goals.

This position is scheduled Monday through Friday. Some evening hours will be required. Qualifications
  • Bachelor's degree in relevant field required. Advanced degree preferred.
  • 3-5 years of senior level fundraising experience including experience managing staff.
  • A proven track record of securing major donor gifts and corporate/individual sponsorships.
  • Familiarity with fundraising for healthcare delivery highly preferred.
  • Familiarity with New York's philanthropic LGBT community.
  • A demonstrated ability to think creatively and bring fresh ideas and approaches to development initiatives.

LPN CASE MANAGER
Position Summary
The Licensed Practical Nurse Case Manager (LPNCM) is responsible for assisting adherence nurses, physicians, and mid-level providers by providing patient education, adherence, and case management support to patients. Reporting directly to the Director of Nursing, the LPNCM will be responsible for assessing the continuity of care for primary care patients with regular review of patients that meet eligible caseload requirements and will be responsible for measuring individual care and treatment adherence. The LPNCM will provide patients with information about their progress toward treatment goals and provide practitioners information about the progress of their patients by creating an adherence plan and overseeing quality assurance of documentation processes around the coordination of care and referrals.

SOCIAL WORKER, HEALTH OUTREACH TO TEENS (HOTT)
Position Summary
We are seeking a dedicated, flexible individual to join our staff as a full time Social Worker within our Health Outreach To Teens (HOTT) program. HOTT provides low- and no-cost medical and mental health care to LGBTQ and homeless youth, ages 13-23. The Social Worker is responsible for providing clinical social work services to difficult to engage youth accessing medical and mental health services within the HOTT program. This includes short-term individual counseling, group counseling, family counseling, crisis management, and psycho-educational services for youth. The Social Worker is responsible for appropriate documentation of care, as well as written mental health assessments and treatment plans. The HOTT Social Worker will work closely with medical, case management, and outreach workers to ensure that the needs of HOTT clients are identified and appropriate services accessed, coordinated, and monitored.

PART-TIME CLINICAL SOCIAL WORKER
Position Summary
In this role you will provide both individual psychotherapy sessions to our adult clients as well as run a support group for newly diagnosed HIV positive men. Interventions include psychotherapy, supportive counseling, crisis intervention, and referral services. The CSW works in collaboration with Callen-Lorde medical staff, consulting psychiatrist, and members of our multi-disciplinary team. The overall goal of the position is to ensure that patients’ psychosocial needs are addressed and met. The CSW is responsible for maintaining on-going documentation in an electronic charting system.

PATIENT NAVIGATOR
Position Summary
Callen-Lorde is seeking a dedicated, flexible individual to join our staff as a Patient Navigator. The Patient Navigator is responsible for providing field-based case management and adherence support services to HIV-positive individuals accessing medical services. The Patient Navigator will work in a team setting headed by a Care Coordinator and in conjunction with medical and mental health providers to ensure that patients maintain stable health. This is a field-based position, working with patients in their homes and accompanying them to appointments, and as such, the schedule is determined by patient needs. The goals of patient navigation are to ensure that HIV-positive patients are linked to care in a timely manner, to maintain patients in care through coordination of medical and social services, and to provide support and coaching. By teaching and supporting treatment/medication adherence, the Patient Navigator will help patients become self-sufficient and able to manage their medical and social needs autonomously.

PATIENT CARE ASSOCIATE
Position Summary
The Patient Care Associates (PCAs) are part of our dynamic medical team and work in collaboration with our nursing and provider staff to ensure optimal patient care. The PCAs are the primary customer service contact with our patients. In this role, you are responsible for maintaining an efficient flow of patients, visitors, and phone calls into the various systems of the Center, providing administrative support to the clinical staff as well as assisting with the collection of patient registration, billing and payment data. You must have a passion for helping others, be courteous, patient and professional at all times. An entry-level position for recent college grads, it is an excellent way to begin a career in a not-for-profit or health care setting.

VOLUNTEERING

Callen-Lorde welcomes volunteers to provide services to our patients and the community.

MEDICAL PROVIDERS
Licensed/certified volunteers, providing direct patient care services, must be credentialed by the relevant Callen-Lorde department. Callen-Lorde welcomes qualified volunteers in the following positions: MDs, NPs, PAs, DOs. Must be NYS licensed and maintain individual malpractice coverage.

Callen-Lorde volunteer medical providers provide episodic care for patients enrolled in adult primary care services. In addition, some providers choose to work within the Sexual Health Clinic, which provides a comprehensive, sex-positive approach to screening, diagnosis, treatment, and prevention of recurrence of sexually transmitted infections (STIs) involving both medical and counseling interventions. All medical provider volunteers work Wednesday evenings under the supervision of a staff physician.
